摘要 目的探讨Notch1和Hes1相关蛋白在膝关节骨性关节炎发生发展中的作用。方法选取2018年3月1日—2019年2月28日于宁夏医科大学总医院骨科行人工全膝关节置换术术中去除的膝骨性关节炎软骨标本32例作为病例组,选取膝关节内骨折切开复位去除的部分软骨组织16例作为对照组。通过番红及HE染色等形态学方法,将病例组分为中期软骨组(9例)及晚期软骨组(23例)。采用免疫组化技术检测Notch1和Hes1蛋白在关节软骨组织中的表达,分析其在软骨中的表达与膝关节骨性关节炎发生发展的关系。结果通过免疫组化可观察到Notch1阳性表达率在对照组软骨组织中为(21.16±7.86)%,在中期软骨组为(33.62±5.87)%,在晚期软骨组为(47.12±4.33)%;Hes 1阳性表达率在对照组软骨组织中为(11.56±5.92)%,在中期软骨组为(27.42±6.27)%,在晚期软骨组为(33.47±4.63)%。随软骨组织病变加重,Notch1和Hes1的阳性表达率相应增高(P均<0.05)。结论Notch1和Hes1相关蛋白参与膝关节骨性关节炎的发生发展,其着色强度随着膝关节软骨的磨损程度的加重而升高,可作为诊断膝关节骨性关节炎的参考指标之一。 Objective To explore the role of Notch1 and Hes1 related proteins in the development of knee osteoarthritis.Methods Thirty-two knee osteoarthritis cartilage specimens removed from artificial total knee arthroplasty in the Department of Orthopedics,General Hospital of Ningxia Medical University from March 1,2018 to February 28,2019 were selected as case group.Sixteen cartilage specimens which were obtained from patients with partial cartilage tissue removed by fracture incision and reduction were used as the control group.According to morphological methods such as saffron and HE staining,the cases were divided into middle cartilage group(9 cases)and advanced cartilage group(23 cases).Immunohistochemistry was used to detect the distribution and expression of Notch1 and Hes1 proteins in osteoarthritis cartilage tissue,and to analyze the relationship between the expression of Notch1 and Hes1 in cartilage and the occurrence and development of osteoarthritis.Results It was observed by immunohistochemistry that the positive expression rate of Notch1 was(21.16±7.86)%in control cartilage tissue,(33.62±5.87)%in middle cartilage group,and(47.12±4.33)%in advanced cartilage tissue.The positive expression rate of Hes1 was(11.56±5.92)%in control cartilage tissue,(27.42±6.27)%in middle cartilage group,and(33.47±4.63)%in advanced cartilage tissue.With the increase of cartilage tissue lesions,the positive expression rate of the two factors increased correspondingly(P all<0.05).Conclusion Notch1 and Hes1 can participate in the occurrence and development of knee osteoarthritis.Its staining intensity increases with the abrasion of knee cartilage,which can be used as one of the reference indicators for the diagnosis of knee osteoarthritis.
