系统性红斑狼疮各证候方药规律临床回顾性调查研究

Clinical retrospective review of syndrome,formula and Chinese medicine laws of systemic lupus erythematosus

目的:用临床回顾性调查探讨系统性红斑狼疮各证候方药规律。方法:收集广州中医药大学第一附属医院2011年1月至2017年5月西医诊断为系统性红斑狼疮的322份病历,进行Logistic多元逐步回归分析。结果:涉及方剂65首、中药236味、中成药47种、西药87种。筛选出有针对治疗意义的方剂和临床用药(P<0.05):热毒炽盛证是犀角地黄汤以及水牛角、喜炎平注射液等,风湿热痹证是桂枝芍药知母汤、四妙散以及三七、替普瑞酮胶囊等,气血两虚证是归脾汤以及麦冬、参附注射液等,肝肾阴虚证是知柏地黄汤、青蒿鳖甲汤以及墨旱莲、生脉注射液等,脾肾阳虚证是金匮肾气丸及呋塞米片、芥子等。结论:Logistic回归分析可用于系统性红斑狼疮各证候的方药规律研究。

Objective:To investigate the syndrome,formula and Chinese medicine laws of systemic lupus erythematosus(SLE)based on clinical retrospective review.Methods:A total of 322 case history of SLE patients from Jan,2011 to May,2017 in The First Affiliated Hospital of Guangzhou University of Chinese Medicine were collected,and analyzed by Logistic regression analysis.Results:A total of 65 prescriptions,236 Chinese medicines,47 proprietary Chinese medicines and 87 western medicines were discovered.The meaningful formulas and medicines(P<0.05)were found,in severe heat poisoning syndrome:Xijiao Dihuang Decoction,Cornu bubali,Xiyanping Injection and so on;in rheumatic heat arthralgia syndrome:Guizhi Shaoyao Zhimu Decoction,Simiao Powder,panax notoginseng,Teprenone Capsules and so on;in qi and blood deficiency syndrome:Guipi Decoction,Radix Ophiopogonis,Shenfu Injection and so on;in liver-kidney yin deficiency syndrome:Zhibai Dihuang Decoction,Qinghao Biejia Decoction,eclipta,Shengmai Injection and so on;in spleen-kidney yang deficiency syndrome:Jingui Shenqi Pills,Furosemide Tablet,India mustard seed and so on.Conclusion:Logistic regression analysis can be used in the researches of the syndrome,formula and Chinese medicine laws of SLE.
