推拿按揉法对坐骨神经慢性压迫性损伤模型大鼠步态及腓肠肌的影响 被引量:9

Effects of kneading manipulation method of massage on gait and gastrocnemius of rats with chronic constriction injury of the sciatic nerve

摘要 目的:通过观察推拿按揉法对坐骨神经慢性压迫性损伤(CCI)模型大鼠右后足步态参数及右侧腓肠肌的影响,探讨推拿按揉法对CCI模型大鼠后肢运动功能的干预作用。方法:将32只雄性SD大鼠随机分为空白组、假手术组、模型组及模型+推拿组,每组8只。模型制备通过坐骨神经结扎造成大鼠神经病理痛。模型+推拿组大鼠在造模后第4天开始给予推拿按揉右侧腓肠肌的干预治疗,连续14d。运用CatWalk三维步态系统观察4组大鼠造模前后不同时间点右后足步态参数的改变,步态测试后观察4组大鼠右侧腓肠肌湿重及形态学的改变。结果:空白组、假手术组各时间点CatWalk步态参数差异无统计学意义;模型组及模型+推拿组在CCI造模后各时间点最大接触面积、足印面积及摆动速度3项步态参数均较空白组显著下降(P<0.01,P<0.05),造模后第14、17天,模型+推拿组这3项步态参数较模型组显著升高(P<0.05);模型组及模型+推拿组在CCI造模后各时间点摆动时间较空白组显著升高(P<0.01,P<0.05),造模后第14、17天,模型+推拿组摆动时间较模型组显著降低(P<0.05)。模型组和模型+推拿组的右侧腓肠肌湿重及恢复率较空白组显著降低(P<0.01),但两组之间差异无统计学意义。模型组腓肠肌细胞横截面积显著小于空白组(P<0.05),模型+推拿组显著大于模型组(P<0.05)。结论:推拿按揉法能改善CCI模型大鼠的后肢步态,延缓腓肠肌的萎缩,证实推拿按揉法能够恢复CCI模型大鼠的后肢运动功能。 Objective:By observing the effect of kneading manipulation method of massgae on the gait parameters of right hind foot and right gastrocnemius muscle of rats with the chronic constriction injury of the sciatic nerve(CCI),the intervention effect of kneading manipulation method of massage on the hindlimb motor function of CCI model rats was discussed.Methods:Thirty-two male SD rats were randomly divided into naive group,sham group,model group and model+Tuina group,with 8 rats in each group.The rat neuropathic pain was induced by sciatic nerve ligation.Rats in the CCI+Tuina group started to receive Tuina treatment on the right gastrocnemius muscle for 14 consecutive days on the fourth day after model establishment.The changes in the gait parameters of the right hindfoot at different time points before and after model establishment were observed using the CatWalk three-dimensional gait system,and the changes in the wet weight and morphology of the right gastrocnemius muscle of the 4 groups of rats were observed after the gait test.Results:There was no CatWalk gait parameters significant change at all time points in the naive group and the sham group.The gait parameters of the model group and the model+Tuina group at each time point after CCI modeling decreased significantly compared with the naive group(P<0.01,P<0.05).On the 14th and 17th day after modeling,the gait parameters of the model+Tuina group increased significantly compared with the model group(P<0.05).The swing time of the model group and the model+Tuina group increased significantly at each time point after CCI modeling(P<0.01,P<0.05),and the swing time of the model+Tuina group decreased significantly compared with the model group(P<0.05)on the 3rd,14th and 17th day after modeling.Wet weight and recovery rate of gastrocnemius muscle were significantly lower in the model group and the model+Tuina group than in the naive group(P<0.01),but there was no significant difference between the two groups.Gastrocnemius cell crosssectional area in the model group was significantly smaller than that in the naive group(P<0.05),and that in the model+Tuina group was significantly larger than that in the model group(P<0.05).Conclusion:Kneading manipulation method of massgae can improve the gait of hind limbs in CCI model rats and delay the atrophy of gastrocnemius muscle.
