全自动磁微粒发光免疫分析法检测梅毒抗体结果分析

摘要 目的对全自动磁微粒发光免疫分析法检测的梅毒抗体阳性结果进行分析,正确地看待梅毒抗体检测对梅毒的诊断价值,减少漏诊误诊。方法分析梅毒抗体阳性者的不同科室来源、年龄、性别;梅毒抗体结果不同定量区间的假阴假阳性;检测灰区的设定。结果TP-Ab阳性率男女相比有统计学意义(P<0.05);60岁以上与30-60岁相比有统计学意义(χ^2=5.4695,P=0.0194);TP-Ab假阳性率60岁以上与30-60岁相比有统计学意义(P<0.05);神经内科、肿瘤科和妇产科梅毒抗体假阳性率排前三位。在0.7≤S/CO<10的区间内,TP-Ab检测结果S/CO值与TPPA总符合率为95.1%,在1≤S/CO<4区间内,阳性符合率仅为12.0%。结论全自动磁微粒发光免疫分析法使大范围地对梅毒筛查变得简便易行,且准确度灵敏度也大大提高,对梅毒的防控起到积极作用。临床梅毒抗体结果的解释,要综合考虑S/CO定量结果范围和患者的临床表现、年龄、病史、原发疾病等,并结合TPPA复检,可以大大减少假阳性的发生,避免误诊漏诊,给患者带来的不必要的痛苦和负担。 Objective To correctly views the diagnostic value of syphilis antibody detection on syphilis,so as to reduce misdiagnosis and missed diagnosis,this paper analyzes the positive results of syphilis antibody by fully automatic chemiluminescent microparticle immunoassay.Methods We analyze different section sources,ages,and genders of syphilis antibody positive people.False negative and false positive in the different quantitative range of results of syphilis antibody.the setting of the detects grey area.Results The positive rate of TP-Ab is statistically significant between men and women(P<0.05)and between over 60 and 30-60 years old(P<0.05);the false positive rates of syphilis antibody in neurology,oncology and obstetrics and gynaecology were the top three;In the range of 0.7≤S/CO<10,the total compliance rates between the TP-Ab detection and the TPPA was 95.1%;in the range of 1≤S/CO<4,the positive compliance rate was only 12.0%.Conclusion The fully automatic chemiluminescent microparticle immunoassay makes it easy to screen syphilis on a large scale,and the accuracy and sensitivity are also greatly improved,which plays a positive role in the prevention and control of syphilis.the interpretation of the clinical syphilis antibody results should comprehensively consider the scope of the S/CO quantitative results and the patient's clinical manifestations,age,medical history,and primary diseases,and combined with the TPPA re-examination,which can greatly reduce the occurrence of false positives and avoid misdiagnosis,unnecessary pain and burden to the patient.
