改良“三步排痰”辅助治疗新生儿肺炎的疗效观察

Observation on effect of improved three-step sputum elimination in adjuvant treatment of neonatal pneumonia

目的:观察改良“三步排痰”辅助治疗新生儿肺炎的疗效。方法:选取我院收治的86例新生儿肺炎患儿为研究对象,以随机数字法分为观察组和对照组,每组43例。对照组予以常规治疗+传统排痰治疗,观察组予以常规治疗+改良“三步排痰”治疗。比较两组患儿的临床疗效,检测两组患儿治疗前后氧分压(PaO 2)和二氧化碳分压(PaCO 2),比较两组患儿排痰量、肺部湿性啰音消退时间、咳嗽消失时间、住院时间等临床指标。结果:观察组的治疗总有效率为81.40%,对照组的治疗总有效率为53.49%,两组比较差异有统计学意义(P<0.05);治疗后,观察组的PaO 2水平高于对照组,PaCO 2水平低于对照组,两组比较差异有统计学意义(P<0.05);观察组的每次排痰量多于对照组,肺部湿性啰音消退时间、咳嗽消失时间短于对照组,两组比较差异有统计学意义(P<0.05),两组在住院时间方面比较,差异无统计学意义(P>0.05)。结论:新生儿肺炎运用改良“三步排痰”辅助治疗可显著改善血气值,增加排痰量,缩短湿啰音、咳嗽消失时间,疗效显著。

Objective:To observe effect of improved three-step sputum elimination in adjuvant treatment of neonatal pneumonia.Methods:86 children with neonatal pneumonia who were admitted to the hospital were enrolled in the study.They were divided into observation group and control group by random number table method,43 cases in each group.The control group was given routine treatment and traditional sputum elimination treatment.The observation group was given routine treatment and improved three-step sputum elimination treatment.The clinical curative effects were compared between the two groups.The partial pressure of oxygen(PaO 2)and partial pressure of carbon dioxide(PaO 2)in both groups were measured before and after treatment.The clinical indexes such as volume of sputum elimination,disappearance time of lung wet rale,disappearance time of cough and hospital stay were compared between the two groups.Results:The total response rates of treatment in the observation group and the control group were 81.40%and 53.49%,respectively(P<0.05).After treatment,PaO 2 level in the observation group was higher than that in the control group,while PaO 2 level was low than that in the control group(P<0.05).the volume of sputum elimination in the observation group was more than that in the control group,while disappearance time of lung wet rale and disappearance time of cough were shorter than those in the control group(P<0.05).There was no significant difference in hospital stay between the two groups(P>0.05).Conclusions:The adjuvant treatment of improved three-step sputum elimination for neonatal pneumonia can improve blood gas value,increase volume of sputum elimination,shorten disappearance time of wet rale and cough.And its curative effect is significant.
