比较血浆置换和胰岛素泵快速降脂方式在急性脂源性胰腺炎治疗中的效果差异

Comparison of the differences in effects of plasma exchange and rapid lipid lowering methods of insulin pump in the treatment of acute lipogenic pancreatitis

目的:对比血浆置换和胰岛素泵快速降脂方式治疗脂源性胰腺炎的效果。方法:2017年8月-2019年7月收治急性脂源性胰腺炎患者261例,随机分为两组。对照组采取胰岛素泵快速降脂方式治疗,观察组采取血浆置换治疗。比较两组治疗效果。结果:观察组患者甘油三酯(TG)达标时间、游离脂肪酸(FFA)正常时间均明显短于对照组,且观察组患者胰腺脓肿、胰腺假性脓肿发生率明显低于对照组,差异均有统计学意义(P<0.05)。结论:两种紧急降脂治疗方案均能达到降脂效果,其中血浆置换TG达标时间、FFA正常时间更短,且胰腺脓肿、胰腺假性脓肿发生率更低。

Objective:To compare the effect of plasma exchange and rapid lipid lowering methods of insulin pump in the treatment of lipogenic pancreatitis.Methods:From August 2017 to July 2019,261 cases of acute lipogenic pancreatitis were enrolled,they were randomly divided into two groups.The control group was treated with rapid lipid lowering methods of insulin pump and the observation group was treated with plasma exchange.The therapeutic effects between the two groups were compared.Results:The compliance time of triacylglycerol(TG)and the normal time of free fatty acids(FFA)in the observation group were shorter than in the control group,the incidence of pancreatic abscess and pancreatic pseudo abscess in the observation group was significantly lower than in the control group,the differences were statistically significant(P<0.05).Conclusion:Two kinds of emergency lipid lowering treatment can achieve the effect of lipid lowering,among them,by plasma exchange,TG reached the standard time and FFA restored normal time is shorter and the incidence of pancreatic abscess and pancreatic pseudo abscess is lower.
