OPD后照法和传统的前节照相法对Toric人工晶状体植入术后患者的视觉质量及旋转稳定性的评价作用 被引量:1

摘要 目的利用OPD后照法及传统的前节照相法评估Toric人工晶状体植入术后患者的视觉质量及旋转稳定性。方法选取术前角膜散光大于1.0 D的白内障患者29例(29眼),行白内障手术并植入Toric人工晶状体。检测术前及术后1周、1个月、3个月患者裸眼视力、全眼像差及人工晶状体位移像差。采用前节照相法、OPD后照法分析患者人工晶状体轴位来评估旋转稳定性。结果本研究患者术后1周、1个月及3个月裸眼视力均较术前升高,差异均有统计学意义(均为P<0.05);术后1个月较术后1周裸眼视力亦升高,差异有统计学意义(t=3.121,P=0.004);术后1个月患者视力已较稳定。术后1周、1个月及3个月患者全眼像差较术前减小,差异均有统计学意义(均为P<0.05)。术后1周前节照相法与OPD后照法均观察到24眼发生旋转。术后1个月前节照相法观察到22眼发生旋转,其中21眼为术后1周时观察到的旋转者,1眼为新观察到的病例;OPD后照法观察到26眼发生旋转,其中24眼与术后1周时观察到的旋转者一致,2眼为新观察到的病例。术后3个月前节照相法观察到22眼发生旋转,OPD后照法观察到26眼,均与术后1个月观察到的旋转者一致。结论Toric人工晶状体植入术后囊袋内稳定性良好,通过OPD后照法检测术后的人工晶状体旋转角度及视觉质量较前节照相法更方便直观、经济准确。 Objective To explore the visual quality and rotational stability after the implantation of Toric intraocular lens(IOL)using OPD posterior segment photographic method and traditional anterior segment photographic method.Methods Totally 29 cataract patients(29 eyes)with corneal astigmatism greater than 1.0 D were enrolled in the study,and received cataract surgery and implantation of Toric IOL.Uncorrected visual acuity(UCVA),total aberration and phase difference caused by IOL displacement were measured before surgery,1 week,1 month and 3 months after surgery.Rotational stability was achieved by anterior segment photographic method and OPD posterior segment photographic method to observe IOL axis.Results The UCVA at 1 week,1 month and 3 months after surgery were higher than that before surgery,and the differences were statistically significant(all P<0.05).The UCVA at 1 month after surgery was higher than that at 1 week after surgery,and the difference was statistically significant(t=3.121,P=0.004).The visual acuity became stable at 1 month after surgery.The total aberration at 1 week,1 month and 3 months after surgery were lower than that before surgery,and the differences were statistically significant(all P<0.05).Rotation was observed in 24 eyes with OPD posterior segment photographic method and traditional anterior segment photographic method at 1 week after surgery.Rotation was observed in 22 eyes at 1 month after the operation using anterior segment photographic method,among of which 21 eyes were those observed at 1 week after surgery,and 1 eye was newly observed case.Rotation was observed in 26 eyes at 1 month after surgery using OPD posterior segment photographic method,of which 24 eyes were consistent with those at 1 week after surgery and 2 eyes were newly observed cases.At 3 months after surgery,22 eyes were observed to rotate by anterior segment photographic method,and 26 eyes were observed by OPD posterior segment photographic method,which were consistent with those observed at 1 month after surgery.Conclusion Toric IOL implantation has a good capsular stability.OPD posterior segment photographic method is good to detect postoperative IOL rotation angle and visual quality,and is more convenient,economical and accurate than anterior segment photographic method.
