危重症患者膀胱温与直肠温、耳温的比较研究 被引量:7

摘要 目的通过比较危重症患者膀胱温与直肠温、耳温的差异性及相关性,探讨膀胱温应用于危重症患者体温监测的可行性。方法2018年1月至2019年5月,便利抽样法选取广州市某三级甲等医院重症监护室(intensive care unit,ICU)收治的47例重症患者为研究对象,分别测量其入科后24h内6个时间点(2:00、6:00、10:00、14:00、18:00、22:00)的膀胱温与直肠温、耳温。采用重复测量方差分析及组内相关系数,比较三种体温测量结果间的差异性及相关性。结果膀胱温与直肠温、耳温的差异均有统计学意义(均P<0.001),与测量时间无交互作用。直肠温(r=0.91,P<0.05)、耳温(r=0.70,P<0.05)与膀胱温呈正相关。结论膀胱温与直肠温、耳温相关性较好,是ICU重症患者监测体温的可靠方法之一;且膀胱温能够持续监测,有助于减轻护士工作量,降低交叉感染发生率,值得在ICU中推广。 Objective To compare the difference and correlation among bladder temperature,rectal tern perature.and ear temperature,and to explore the feasibility of testing bladder temperature in critically ill patients.Methods By convenience sampling method,47 critically ill patients that hospitalized in ICU from January 2018 to May 2019 were recruited.Bladder temperature,rectal temperature and ear temperature were measured at 6 time points in the first 24h of admission(2:00,6:00,10:00,14:00,18:00,22:00),respectively.Repeated measurement analysis of variance and intraclass correlation coefficient were used to analysis the differences and correlations among the three body temperature measurements.Results There was significant difference among rectal temperature,ear temperature and bladder temperature(all P<0.001),but there was no interaction with measuring time.Rectal temperature(r=0.91.P<0.05),and ear temperature(r=0.70,P<0.05)had a high positive correlation with bladder temperature.Conclusions Bladder temperature has a high correlation with rectal temperature,ear temperature,which is one of the reliable methods to monitor body temperature in severe ICU patients.Bladder temperature can be monitored continuously,reduce the workload of nurses,and reduce the incidence of cross infection,which is worth popularizing in ICU.
