ADAMTS4与ADAMTS5蛋白酶在胶质瘤中表达的相关性

ADAMTS4 and ADAMTS5 protease expressions in glioma

目的分析含血小板结合蛋白基序的解聚蛋白样金属蛋白酶(ADAMTS)4与ADAMTS5蛋白酶在胶质瘤中表达及其同患者病情、预后的相关性。方法选取作者医院神经外科自2016年1月至2017年1月收治的116例脑胶质瘤患者,收集患者经手术切除或穿刺采集的胶质瘤病灶组织,以免疫组织化学检测ADAMTS4与ADAMTS5蛋白酶的表达率,比较不同分级、瘤周水肿程度以及是否伴囊变患者的病灶组织中ADAMTS4与ADAMTS5蛋白酶的表达率差异,采用Spearman分析方法分析两种蛋白酶表达率的相关性,并比较ADAMTS4阳性、阴性表达与ADAMTS5阳性、阴性表达的累积生存时间差异。结果所有患者病灶组织中ADAMTS4、ADAMTS5的表达率为79.31%(92例)、84.48%(98例)。不同分级患者的病灶组织中ADAMTS4与ADAMTS5蛋白酶的表达率有统计学意义,Ⅳ级、Ⅲ级患者病灶组织中ADAMTS4与ADAMTS5蛋白酶表达率明显高于Ⅰ级、Ⅱ级者(P<0.05);伴重度瘤周水肿患者的病灶组织中ADAMTS4与ADAMTS5蛋白酶的表达率明显高于伴轻度瘤周水肿者(P<0.05);伴囊变和未伴囊变者的病灶组织中ADAMTS4与ADAMTS5蛋白酶的表达率无统计学差异(P>0.05);Spearman分析结果显示两种蛋白酶表达率成正相关(r=0.361,P=0.001);ADAMTS4、ADAMTS5阳性表达者的累积生存率明显低于阴性表达者(P<0.05)。结论胶质瘤的分级、瘤周水肿程度越高者病灶细胞中ADAMTS4、ADAMTS5蛋白酶的表达更高,两种蛋白酶可能存在协同效应,参与脑胶质瘤的病情进展、病灶浸润。

Objective To analyze the expressions of ADAMTS4 and ADAMTS5 protease in glioma and their correlation with the patient's condition and prognosis.Methods 116 patients with gliomas diagnosed at the Department of Neurosurgery from January 2016 to January 2017 were selected.Immunohistochemistry was used to detect the expressions of ADAMTS4 and ADAMTS5 proteases in the tissue of glioma lesions collected by surgical excision or puncture.The expression rates of ADAMTS4 and ADAMTS5 in the lesions with different grades,peritumoral edema,and cystic degeneration were compared.Spearman’s correlation was used to analyze the correlation of ADAMTS4 and ADAMTS5 proteases’expressions.The cumulative survival time of ADAMTS4 and ADAMTS5 positive and negative patients were compared.Results The expression rates of ADAMTS4 and ADAMTS5 in the lesions were 79.31%(92 cases)and 84.48%(98 cases).The expression rates of ADAMTS4 and ADAMTS5 proteases in the lesions of different grades were statistically significant(P<0.05).The expression rates of ADAMTS4 and ADAMTS5 in tissues with severe peritumoral edema were significantly higher than those with light peritumoral edema(P<0.05).Spearman analysis showed that expression rates of ADAMTS4 and ADAMTS5 were positively correlated(P<0.05).The cumulative survival rate of ADAMTS4 and ADAMTS5 positive patients were significantly lower than those with negative expressions(P<0.05).Conclusions The higher the grade and the more severe the peritumoral edema of glioma,the higher the expression of ADAMTS4 and ADAMTS5 in the peritumoral edema lesion cells.The two proteases may have synergistic effect and participate in the progression and infiltration of glioma.
