肝癌血管性介入治疗临床主要有3种治疗方式,分别为肝动脉灌注治疗(TAI)、肝动脉栓塞(TAE)和肝动脉化疗栓塞(TACE)。3种治疗方式的区别在于肝动脉灌注治疗主要是通过导管向动脉内灌注化疗药物;肝动脉栓塞主要是通过导管将明胶海绵、碘化油等栓塞剂注入肿瘤供血动脉;肝动脉化疗栓塞则是经导管既灌注化疗药物,又使用栓塞剂,目前在临床广泛应用。在编码肝癌血管性介入治疗的操作时,首先要通读操作记录来区分本次介入治疗的方式,明确是单纯灌注化疗药物还是物理性栓塞,或者是两者同时进行,选择性肝动脉灌注治疗一般需要3个编码,分别为99.25注射或输注癌瘤化学治疗药物,50.93肝局部灌注和88.47其他腹内动脉造影术(肝动脉造影)。选择性肝动脉栓塞一般需要2个编码,分别为39.79为其他血管的其他血管内修补术(肝动脉栓塞术)和88.47其他腹内动脉造影术(肝动脉造影)。选择性肝动脉化疗栓塞一般需要4个编码,分别为99.25注射或输注癌瘤化学治疗药物,50.93肝局部灌注(灌注部位),39.79为其他血管的其他血管内修补术(肝动脉栓塞术)和88.47其他腹内动脉造影术(肝动脉造影)。编码时注意不要遗漏附加编码和另编码。
