摩擦性苔藓样疹546例临床分析

Clinical analysis of 546 cases with frictional lichenoid eruptions

目的:探究摩擦性苔藓样疹发病季节、发病原因、皮损特点、病程进展、不同药物治疗疗效、预后及转归,以期探讨本病是否与过敏相关。方法:临床资料采集以门诊问卷调查方式,随访以门诊复诊和电话随访方式。结果:在546例患儿中88.28%有接触史。63例患儿中斑贴试验阳性有30例,阳性率为47.6%;0.1%糠酸莫米松组与0.03%他克莫司组治疗疗效无明显差异。结论:摩擦性苔藓样疹病因可能与直接摩擦刺激及接触物所致过敏有关,糖皮质激素治疗与钙调神经磷酸酶抑制剂治疗本病疗效相当。

Objective: To explore the seasonal onset, pathogenesis, characteristics of the skin lesions, disease course, therapeutic efficacy as well as prognosis of patients with frictional lichenoid eruptions. To explore if the frictional lichenoid eruptions is associated with other allergic skin diseases or it may represent a certain stage of skin lesions. Methods: The clinical data were collected from questionnaires of outpatient visits, and clinical follow up was done during outpatient re-visit and telephone interviews. Results: A total of 546 children with frictional lichenoid eruption, 88.28% of which had a history of exposure to allergens. Thirty of the 63 children had a positive patch test, with a positive rate of 47.6%. There was no significant difference in the therapeutic efficacy between 0.1% mometasone furoate treated group and 0.03% tacrolimus treated group.Conclusion: The pathogenesis of the disease may be related to direct contact stimulation and allergic reaction to the contactant.Treatment with glucocorticoid and caicineurin inhibitor showed an equivalent curative effect on the disease.
