口腔颌面间隙感染320例临床分析

Clinical Analysis of 320 Cases of Oral and Maxillofacial Infection

目的分析口腔颌面间隙感染临床特征,总结管理经验。方法2005年1月至2018年12月,医院共收治了口腔颌面间隙感染320例,回顾性分析。结果肿胀及疼痛最多见,部分患者伴有张口受限,咽部肿痛,少数患者伴有发热,个别患者伴有吞咽障碍及呼吸困难,牙源性占70.3%、腮腺源19.7%,多间隙感染16.88%,常见的间隙包括下颌下间隙38.75%,累及颈部间隙3.1%。合并糖尿病173例,星座链球菌39.1%,72.50%单纯的抗生素治愈,8例自然排脓,其余对象切开引流。无严重并发症病例、无死亡例。糖尿病患者男性比重、牙源性比重、中性粒细胞上升、高血糖率高于无糖尿病对象,单纯抗生素治愈率低于无糖尿病对象,差异有统计学意义(P<0.05)。结论口腔颌面间隙感染有自身的特点,严重并发症发生情况与感染的部位有关,糖尿病对疾病影响较大,多见于中老年人,此类对象的严重并发症发生率高,需要加强管理,积极控制血糖。

Objective To analyze the clinical features of oral and maxillofacial infection and summarize management experience.Methods From January 2005 to December 2018,the hospital treated 320 cases of oral and maxillofacial space infection,retrospective analysis.Resluts Swelling and pain were most common in some patients with limited opening,swelling and pain in the pharynx,fever in a few patients,dysphagia and dyspnea in a few patients,odontogeny accounted for 70.3%,parotid gland source was 19.7%and multispace infection was 16.88%.Common spaces include the submandibular space of 38.75,involving the neck between 3.1.Among 173 cases of diabetes mellitus,72.50%of streptococcus constellans were cured with antibiotics alone,8 cases with spontaneous discharge of pus,and others with incision and drainage.There were no serious complications and no death.Proportion of male,odontogenic,neutrophilic granulocytes in diabetic patients The rate of hyperglycemia was higher than that of non-diabetic subjects,and the cure rate of antibiotics was lower than that of non-diabetic subjects(P<0.05).Conclusion Oral and maxillofacial infections have their own characteristics.The occurrence of serious complications is related to the location of infection.Diabetes has a greater impact on the disease.It is more common in middle-aged and elderly people.The incidence of serious complications in such subjects is high and needs to be strengthened.Actively control blood sugar.
