摘要
目的:探讨对比剂分段推注-单期扫描法在肝动脉及门静脉多层螺旋CT成像中的应用。方法:选取在医院诊治的120例行肝动脉及门静脉CT成像的患者,按扫描参数相同而方法不同将其分为对照组(30例)和观察组90例。对照组应用常规两期扫描法,观察组采用分段式推注对比剂、单期扫描。根据扫描时间的不同进一步将观察组分为50 s、55 s和60 s的3个亚组,每组30例。测量各组腹主动脉及门静脉主干、左右支CT值,判断其成像效果,以比较观察组中各组及其与对照组的扫描时间及血管CT参数值。结果:对照组门静脉主干CT值为(160.5±30.3)HU,右支(157.8±28.1)HU,左支(156.5±26.3)HU,55 s组门静脉主干CT值为(239.4±39.7)HU,右支(219.8±32.3)HU,左支(210.1±28.1)HU,两组比较差异均有统计学差异;对照组腹主动脉CT值为(361.2±75.3)HU,55 s组为(356.3±98.1)HU,两组比较无统计学差异。观察组1次成像得到了对照组需2次扫描获取的图像,其受辐射剂量为对照组1/2。结论:对比剂分段式推注-单期扫描能有效地显示腹主动脉及门静脉,能够明显降低患者检查所受辐射剂量,并可获得最佳的血管显像,满足诊断需求。
Objective: To investigate the application of segmented injection of contrast agent(CA)-single phase scan in multi-slice spiral computed tomography(CT) imaging for hepatic artery and portal vein. Methods: A total of 120 patients who underwent hepatic arteriovenous CT imaging were divided into control group(30 cases) and observation group(90 cases) according to different scan methods with same scan parameter. The observation group applied routine scan method with two phases, and observation group adopted segmented injection of contrast agent(CA)-single phase. And then the observation group was further divided into 3 subgroups(50 s subgroup, 55 s subgroup and 60 s subgroup) as different scan time, with 30 cases in each group. The CT values of abdominal aorta, left branch, right branch and trunk of portal vein in each group were measured, and the imaging effect of each group was judged so as to compare the scan time and CT parameter value of blood vessel among different subgroup of observation group and among these subgroups and control group, respectively. Results: The CT values of trunk, right branch and left branch of portal vein were 160.5±30.3 Hu, 157.8±28.1 Hu and 156.5±26.3 Hu respectively in control group. And these were 239.4±39.7 Hu, 219.8±32.3 Hu and 210.1±28.1 Hu in 55 s subgroup respectively. The difference of these CT values between control group and 55 s subgroup was significant. And the CT value of abdominal aorta of control group and 55 s subgroup were 361.2±75.3 Hu and 356.3±98.1 Hu, respectively, and the difference of this CT value between the two group was no significant. And the results from one-time imaging of observation group achieved the effect of twotimes scan of control group, while the radiation dosage of observation group was a half of radiation dosage of control group. Conclusion: The CA segmented-injection-single phase scan can effectively display abdominal aorta and portal vein. And it can significantly reduce the radiation dosage that patients received and obtain optimal angiography of blood vessel, and it can meet the requirement of diagnosis.
