高频和低频超声检查在阑尾炎间接超声征象中的诊断价值分析

Analysis on the Diagnostic Value of High-Frequency and Low-Frequency Ultrasound Examinations in Indirect Ultrasound Signs of Appendicitis

目的分析高频和低频超声检查在阑尾炎间接超声征象中的诊断价值。方法选择2017年7月至2019年7月我院收治的96例疑似阑尾炎患者,所有患者均给予高频超声和低频超声检查,以手术或者后续治疗确定的诊断结果作为金标准,观察高频超声、低频超声、高频+低频超声诊断阑尾炎的灵敏度和特异度,并比较高频超声、低频超声对阑尾炎间接征象的检出率。结果高频+低频超声诊断阑尾炎的灵敏度和特异度显著高于单独诊断(P<0.05)。高频超声对回盲部水肿、阑尾肿大增粗、阑尾壁增厚等间接征象的检出率显著高于低频超声(P<0.05)。结论高频超声对阑尾炎间接超声征象的检出率高于低频超声,两种频率超声联合应用能够显著增加阑尾炎诊断的灵敏度和特异度。

Objective To analyze the diagnostic value of high-frequency and low-frequency ultrasound examinations in indirect ultrasound signs of appendicitis.Methods 96 patients with suspected appendicitis in our hospital from July 2017 to July 2019 were selected,and all patients were examined by high-frequency ultrasound and low-frequency ultrasound.With the diagnosis results determined by operation or follow-up treatment as the gold standard,the sensitivity and specificity of high-frequency ultrasound,low-frequency ultrasound,and high-frequency+low-frequency ultrasound in the diagnosis of appendicitis were observed.The detection rates of indirect signs of appendicitis were compared between high-frequency ultrasound and low-frequency ultrasound.Results The sensitivity and specificity of high-frequency+low-frequency ultrasound in the diagnosis of appendicitis were significantly higher than those of single diagnosis(P<0.05).The detection rates of indirect signs including edema of ileocecal,appendiceal enlargement and thickening of appendix wall by high-frequency ultrasound were significantly higher than those by low-frequency ultrasound(P<0.05).Conclusions High-frequency ultrasound has higher detection rates of indirect signs of appendicitis than low-frequency ultrasound.The combination of high-frequency ultrasound and low-frequency ultrasound can significantly increase the sensitivity and specificity of appendicitis diagnosis.
