“分级诊疗”制度下徐州地区部分医院影像设备配置现状及相关分析

The current situation and relevant analysis of image equipment configuration of some hospitals in Xuzhou under the “Hierarchical diagnosis and treatment”system

目的评价徐州地区部分医院X线机(普通/移动式X线机、直接数字化X射线摄影系统、胃肠机)、CT、MRI影像设备配置的现状,为有关部门实现优化资源配置,有力推动分级诊疗制度的实施提供依据与建议。方法采用SPSS21. 0对数据进行方差分析和t检验。结果各级医院在X线机、CT、MRI的设备来源与品牌方面不同;除基层医院与二级医院在CT配置数量不存在统计学差异(P值0. 052> 0. 05),其余各级医院之间的三种设备均存在明显统计学差异(P <0. 05);各级医院在X线机的日工作量上均存在明显统计学差异(P <0. 05),CT和MRI工作量无统计学差异(sig>0. 05,P>0. 05);除基层医院和二级医院在X线机购置价格上无统计学差异(P值0. 405>0. 05)、二级医院和三级医院在CT购置价格上无统计学差异(sig值0. 142> 0. 05)外,其余各级医院在三种设备的购置价格上均存在明显统计学差异(P <0. 05);各级医院三类影像设备的运行状况均处于良好状态。结论徐州地区各级医院的影像设备差距较大,医疗资源的配置有待完善,基层医院在影像设备上处于落后,难以满足当今医疗事业发展的需求,分级诊疗制度的推进需要克服硬件阻力。

Objective To evaluate the current situation of general/mobile X-ray machine,direct digital X-ray radiography system( DR),CT and MRI imaging equipment configuration in some hospitals in Xuzhou area,so as to provide a basis and suggestions for relevant departments to optimize resource allocation and effectively promote the implementation of hierarchical diagnosis and treatment system. Methods Spss21. 0 was used to analyze the data. Results Hospitals at different levels have different X-ray machine,CT,and MRI equipment sources and brands. Except that there is no statistical difference in the number of CT configurations between primary-level hospitals and secondary hospitals( P value 0. 052 > 0. 05),there is a significant statistical difference in these types of equipment( P < 0. 05);there is a significant statistical difference in the daily workload of X-ray machines at all levels of hospitals( P < 0. 05),and there is no statistical difference in the workload of CT and MRI( sig > 0. 05,P > 0. 05);Except that there is no statistical difference in the purchase price of X-ray machines between primary and secondary hospitals( P value 0. 405 > 0. 05),there is no statistical difference in the purchase price of CT between secondary and tertiary hospitals( sig value 0. 142 > 0. 05). There are obvious statistical differences in the purchase prices of the three types of equipment in the first-level hospitals( P < 0. 05);the operation status of the three types of imaging equipment in all levels of hospitals is in good condition. Conclusion There is a large gap between the imaging equipment of hospitals at all levels in Xuzhou,and the allocation of medical resources needs to be improved,and the basic-level hospitals are lagging behind in the imaging equipment,which is difficult to meet the needs of the development of today’s medical industry. The promotion of the hierarchical diagnosis and treatment system needs to overcome the hardware backwardness.
