PDCA循环法在降低门诊药房药品调配差错中的应用

Application of PDCA cycle to reduce drug dispensing errors in outpatient pharmacy

目的采用PDCA循环法降低门诊药房药品调配差错,提高药品调剂质量。方法统计我院门诊药房2016年10月至2017年2月发生的出门差错(外差)作为改善前数据,采用PDCA循环法,分析发生差错的原因,制定对策,比较改善前、中、后的数据来判定对策的有效性。结果运用PDCA循环的方法管理后,通过采取改进电脑显示、加强相似药品管理和规范药师操作流程等方法,药品调配外差月发生率得到了明显降低。结论运用PDCA循环法可以在一定程度上降低门诊药房药品的调配差错。

Objective To use the PDCA cycle method to reduce drug dispensing errors in outpatient pharmacy and improve drug dispensing quality. Methods The external errors occurred in the outpatient pharmacy of our hospital from October 2016 to February 2017 was counted as the data before the improvement,and PDCA cycle method was adopted to analyze the causes of errors,formulate countermeasures,and compare the data before and after the improvement to determine the effectiveness of countermeasures. Results After using PDCA cycle,the monthly incidence of external errors in drug dispensing was significantly reduced by improving computer display,strengthening similar drug management and standardizing pharmacists’ operating procedures. Conclusion The PDCA cycle management can reduce drug dispensing error in outpatient pharmacy to some extent.
