摘要
目的分析多发性原发性食管癌以适形放疗为主的综合治疗疗效及预后影响因素。方法收集以适形放疗为主进行综合治疗的64例多发性原发性食管癌患者的临床资料,比较不同癌灶情况和临床特征多发性原发性食管癌患者的预后情况,并分析其总体生存情况。结果所有患者的1、2、3年无局部复发生存率分别为56.3%、29.7%、18.8%;无远处转移生存率分别为51.6%、29.7%、18.8%;无疾病进展生存率分别为51.6%、26.6%、18.8%;总生存率分别为56.3%、32.8%、18.8%;中位生存期为24个月。不同年龄、长期饮酒、放疗方式、主癌灶部位的多发性原发性食管癌患者的总生存情况比较,差异均有统计学意义(P﹤0.05)。结论年龄、长期饮酒、放疗方式、主癌灶部位可能与多发性原发性食管癌患者的总生存情况有关。对于多发性原发性食管癌,应根据不同的患者制订个体化的综合治疗方案。
Objective To investigate the efficacy and prognostic factors of conformal radiotherapy-based comprehensive therapy for multiple primary esophageal cancer.Method The clinical data of 64 patients with multiple primary esophageal cancer undergoing conformal radiotherapy-based comprehensive therapy were retrospectively analyzed,the prognosis of patients with different clinical characteristics and cancer lesions were compared and their overall survival was analyzed.Result In this cohort of patients,the 1-,2-,and 3-year local progression-free survival(LPFS)rates were 56.3%,29.7%and 18.8%,distant metastasis-free survival(DMFS)rates were 51.6%,29.7%and 18.8%,progression-free survival(PFS)rates were 51.6%,26.6%and 18.8%;besides,the overall survival(OS)rates were 56.3%,32.8%and 18.8%,respectively,with a median survival time(MST)of 24 months.Univariate analysis showed that,age,long-term alcohol consumption,radiation treatment approach,site of primary lesions were closely related to the overall survival of patients with multiple primary esophageal cancer(P<0.05).Conclusion Age,long-term alcohol consumption,radiation treatment approach,site of primary lesions are associated with the overall survival of patients with multiple primary esophageal cancer.In dividualized comprehensive treatment regimens should be developed for these patients.
