支气管动脉灌注化疗栓塞术联合静脉化疗治疗肺鳞癌的临床应用

Clinical application of bronchial artery infusion chemotherapy and embolization combined with intravenous chemotherapy for lung squamous cell carcinoma

目的:探讨支气管动脉灌注化疗栓塞联合静脉化疗治疗肺鳞癌的临床疗效及安全性。方法:收集35例经支气管动脉灌注化疗栓塞术联合静脉化疗治疗的肺鳞癌患者的临床资料,其中男24例,女11例;周围型12例,中央型23例。总结其肿瘤的血供特点、近期疗效、1年和2年生存率。结果:中央型肺癌的肿瘤染色较周围型肿瘤稍明显。病灶直径>30 mm者富血供率90.3%,直径≤30 mm者富血供率50.0%。35例患者中完全缓解(CR)5例,部分缓解(PR)23例,稳定(SD)5例,进展(PD)2例,肿瘤有效控制率为80.0%。1年生存率为63.6%,2年生存率为54.5%。结论:支气管动脉化疗栓塞术联合全身静脉化疗治疗肺鳞癌,临床疗效确切,可缓解临床症状,延长生存时间。

Objective:To evaluate the clinical efficacy and safety of transcatheter arterial chemoembolization combined with intravenous chemotherapy in the treatment of lung squamous cell carcinoma.Methods:All 35 patients with lung squamous cell carcinoma were treated with bronchial arterial chemoembolization combined with intravenous chemotherapy.There were 24 males,11 females,12 were peripheral lung cancer,and 23 were lung cancer.The blood supply characteristics,short-term curative effect,1 year and 2 year survival rate were summarized.Results:The tumor blood supply of central lung cancer was more pronounced than peripheral tumors.The rich blood supply rate of lesions(>30 mm)is 90.3%,and the rate of≤30 mm is 50.0%.Of the 35 patients,5 were complete remission(CR),23 were partial remission(PR),5 were stable disease(SD),and 2 were progressive disease(PD).The effective tumor control rate was 80.0%.The 1-year survival rate was 63.6%,and the 2-year survival rate was 54.5%.Conclusion:Bronchial arterial chemoembolization combined with venous chemotherapy for lung squamous cell carcinoma has a clinically effective effect,relieves clinical symptoms,prolongs survival time.
