体外冲击波治疗糖尿病合并肩周炎的效果和安全性评估

The clinical efficiency and safety of extracorporeal shock wave therapy in treatment of diabetes mellitus complicated by periarthritis of shoulder

目的评估体外冲击波治疗(extracorporeal shock wave therapy,ESWT)糖尿病合并肩周炎的临床效果与安全性。方法选择2017年1月起至2018年6月的53例糖尿病合并肩周炎患者参与此项研究。其中体外冲击波组(观察组,27例患者27肩)给予冲击波治疗6周,对照组(26例患者26肩)为单纯传统治疗组。治疗后对2组患者均随访12周,采用Constant-Murley评分表评估肩关节功能状态,VAS(视觉模拟评分)评估肩关节治疗前后的疼痛情况,时间节点分别为:治疗前、治疗后2周、6周和12周。采用Student-t检验评估治疗前后以及各时间节点2组肩关节功能评分的差异。结果同治疗前相比,2组肩关节功能评分在治疗结束后的各时间节点均有明显改善,差异有统计学意义(P<0.05)。相较于传统治疗组,体外冲击波治疗组肩关节功能评分在治疗后的各时间节点均较好,差异有统计学意义(P<0.05)。在治疗过程中,2组均未发生任何并发症以及患者主观不适。结论在治疗糖尿病合并肩周炎时,体外冲击波治疗具有良好的安全性和临床效应,可以考虑其作为治疗此类疾病的常规治疗方法。

Objective To evaluate the clinical efficiency and safety of extracorporeal shock wave therapy(ESWT)in treatment of diabetes mellitus complicated by periarthritis of shoulder.Methods A total of 53 patients with diabetes mellitus complicated by periarthritis of shoulder who were treated in our hospital from January 1,2017 to June 30,2018 were enrolled in the study,who were divided into observation group(n=27,27 shoulders)and control group(n=26,26 shoulders).The patients in observation group were treated by extracorporeal shock wave,however,the patients in control group were treated by conventional therapy,with a treatment course of 6 weeks for both groups.After treatment the patients in both groups were followed up for 12 weeks,and the status of shoulder joint function was evaluated according to constant-murley scale before treatment,at 2w,6w,12w after treatment.Results As compared with those before treatment,the scores of shoulder joint function in both groups were significantly improved at different time points after treatment(P<0.05),moreover,which in observation group were superior to those in control group(P<0.05).And no complications or subjective discomfort were found during treatment in both groups.Conclusion The extracorporeal shock wave in treatment of diabetes mellitus complicated by periarthritis of shoulder has better safety and clinical effects,therefor,it can be regarded as a routine therapy for such diseases.
