二维斑点追踪技术评价健心颗粒对糖尿病心肌病患者左室功能的影响 被引量:5

摘要 目的采用二维斑点追踪技术评价健心颗粒对糖尿病心肌病(DCM)患者心室功能的影响。方法选择2017年1月至2019年12月于本院心血管内科新确诊的80名DCM患者为研究对象,分为观察组和对照组,每组40例,两组均给予常规西药治疗方案,观察组在此基础上给予健心颗粒,两组疗程为1个月。比较两组患者在治疗前后临床症状、收缩压、舒张压、左室收缩期整体长轴峰值应变、整体圆周峰值应变、左室射血分数、NT-proBNP等指标变化情况。结果观察组临床疗效总有效率(85.0%)明显高于对照组(60.0%)。治疗后,两组空腹血糖及NT-proBNP均显著减低,但观察组NT-proBNP、射血分数、左室收缩期整体长轴(圆周、径向)峰值应变改善情况明显大于对照组。两组间LA、LVDd、IVSd、LVPWd、E/A治疗前后差异无统计学意义(P>0.05)。结论健心颗粒在改善DCM左室结构前,已明显改善其心室功能;二维斑点追踪技术能够早期准确地对治疗效果进行评价,值得临床推广及应用。 Objective The effect of jianxin granules on ventricular function in patients with diabetic cardiomyopathy(DCM) was evaluated by two-dimensional spot tracking technique.Methods A total of 80 newly diagnosed DCM patients from the department of cardiology, the Second People’s Hospital Affiliated to Fujian University of Traditional Chinese Medicine from January 2017 to December 2019 were selected as subjects.They were divided into the experimental group and the control group, with 40 participants in each group.Both groups were given the conventional western medicine treatment plan, and the experimental group was given jianxin granules on this basis.The treatment course of the two groups was 1 month.The changes of clinical symptoms, systolic blood pressure, diastolic blood pressure, left ventricular systolic overall long-axis peak strain, overall peripheral peak strain, left ventricular ejection fraction, nt-probnp and other indicators in the two groups before and after treatment were compared.Results The total effective rate(85.0%) of the experimental group was significantly higher than that of the control group(60.0%).After treatment, fasting glucose and nt-probnp were significantly reduced in both groups, but the experimental group nt-probnp, ejection fraction, left ventricular systolic overall long axis(circumference, radial) peak strain improvement was significantly greater than the control group.There were no significant differences in LA, LVDd, IVSd, LVPWd and E/A between the two groups before and after treatment.Conclusion Before improving the left ventricular structure of DCM, jianxin granules had significantly improved its ventricular function.The two-dimensional spot tracking technology can accurately evaluate the treatment effect in the early stage, which is worthy of clinical promotion and application.
