摘要
目的了解成都社区T2DM病人的衰弱现状,探索T2DM合并衰弱的主要危险因素。方法采用横断面调查法对2017年3~7月成都社区累计招募的237例T2DM病人进行综合评估。采用Logistic回归分析探讨T2DM合并衰弱的相关影响因素。结果 T2DM病人衰弱患病率为5. 1%,衰弱前期患病率为41. 4%;多因素Logistic回归分析结果显示:T2DM病人学历越高(OR=0. 36,95%CI:0. 14~0. 92)、睡眠质量越好(OR=0. 37,95%CI:0. 17~0. 79)、抑郁评分越低(OR=1. 25,95%CI:1. 08~1. 44)、营养状况越好(OR=0. 70,95%CI:0. 56~0. 87),其衰弱发生的风险越低(P <0. 05)。结论成都社区T2DM病人中,衰弱前期病人较多,宜早期进行预防干预;营养不良、抑郁、睡眠障碍、文化水平低是T2DM合并衰弱综合征的独立危险因素。
Objective To investigate the frailty epidemiological characteristics among the patients with type 2 diabetes mellitus(T2DM)in Chengdu community,and to explore the risk factors of T2DM complicated with frailty. Methods A total of 237 T2DM patients from March to July 2017 in Chengdu community were included in the study. The related risk factors of T2DM complicated with frailty were analyzed by Logistic regression model. Results The motality rate of frailty and pre-frailty among T2DM patients in Chengdu community was 5. 1% and 41. 4%. Multivariate regression analysis showed that the education level(OR=0. 36,95%CI:0. 14-0. 92),sleep quality(OR=0. 37,95%CI:0. 17-0. 79),depression score(OR=1. 25,95%CI:1. 08-1. 44)and nutritional status(OR=0. 70,95%CI:0. 56-0. 87)were risk factors of frailty(P < 0. 05). Conculsions There are many T2DM patients presenting with pre-frailty in Chengdu community. Malnutrition,depression,sleep disorder and low educational level are independent risk factors of frailty. Early prevention and intervention is important for these high-risk population.
