HFNC和NPPV在ICU气管镜检查患者中的应用

APPLICATION OF HFNC AND NPPV IN PATIENTS WITH ICU BRONCHOSCOPY

目的探讨无创通气(NPPV)与经鼻高流量鼻塞(HFNC)两种呼吸支持方式在ICU气管镜检查患者中的应用效果比较。方法142例ICU中床旁行支气管镜的检查,其中71例在进行纤支镜时使用HFNC进行呼吸支持,71例进行NPPV呼吸支持,比较两种呼吸支持方式的吸入氧浓度分数(FIO2)、经皮(PO2/PCO2)、一次性通过率、患者满意度评分、手术时间以及不良反应发生率。结果二组患者的PO2/PCO2、FIO2在呼吸支持过程中差异无统计学意义(P>0.05);二者气管镜一次性通过率均为95%;二者不良反应发生率比较差异无统计学意义(P>0.05);但HFNC组手术时间明显缩短,患者满意度更高,差异有统计学意义(P<0.05)。结论HFNC和NPPV在ICU气管镜检查患者中呼吸支持没有明显差别,但是HFNC组检查时间更短,患者满意度更高。

Objective To investigate the effect of two kinds of respiratory support methods,NPPV and HFNC,in patients with ICU bronchoscopy.Methods A total of one hundred and forty-two ICU patients in the Sichuan Provincial People′s Hospital were examined by bedside bronchoscopy.Among them,71 patients underwent HFNC respiratory support during the bronchoscopy and 71 patients underwent NPPV respiratory support.FIO2,PO2/PCO2,one-time pass rate,patient satisfaction score and operative time of the two respiratory support method were compared.Results There was no significant difference in PO2,PCO2 and FIO2 between the two groups(P>0.05).The one-time pass rate of bronchoscopy was 95%.There was no statistically significant difference in adverse reaction rate(P>0.05);however,the operative time of the HFNC group was significantly shortened,and the patients’satisfaction was higher,where the difference was statistically significant(P<0.05).Conclusion There is no significant difference in HFNC and NPPV in patients with ICU bronchoscopy.However,the HFNC group has shorter operative time and higher patient satisfaction.
