低温等离子下鼻甲射频消融术对慢性鼻-鼻窦炎患者TNF-α、IL-6的影响

Effect of low Temperature Plasma Lower Turbinate Radiofrequency Ablation on TNF-α and IL-6 in Patients with Chronic Rhinosinusitis

目的探讨低温等离子下鼻甲射频消融术对慢性鼻-鼻窦炎患者肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)的影响。方法回顾性分析2015年5月至2019年5月在北京市房山区良乡医院耳鼻喉科治疗的200例慢性鼻-鼻窦炎患者的临床资料,按手术方式不同分为研究组与参照组,各100例。参照组用鼻内镜鼻窦手术,研究组用鼻内镜鼻窦手术联合低温等离子下鼻甲射频消融术。比较2组手术前、手术30 d后TNF-α、IL-6水平及鼻腔鼻窦结局测试-20(SNOT-20)评分量表、鼻腔功能(Lund-kennedy)量表评分;比较2组手术30 d后临床疗效、视觉模拟评分法(VAS)评分评估2组疼痛程度;比较2组手术后并发症发生情况。结果与手术前比,手术30 d后2组SNOT-20、Lund-kennedy评分及血清TNF-α、IL-6水平均降低,且研究组低于参照组(P<0.05)。研究组手术30 d后VAS评分低于参照组(P<0.05)。手术30 d后,研究组总有效率(96.00%)高于参照组(81.00%)(P<0.05)。手术后研究组并发症总发生率(1.00%)低于参照组(7.00%)(P<0.05)。结论低温等离子下鼻甲射频消融术可降低慢性鼻-鼻窦炎患者血清TNF-α、IL-6水平,改善患者鼻腔功能,疗效显著,且不增加并发症发生率,值得推广应用。

Objective To investigate the effects of low temperature plasma lower turbinate radiofrequency ablation on TNF-α and IL-6 in patients with chronic rhinosinusitis.Methods The clinical data of 200 patients with chronic rhinosinusitis treated in the otolaryngology department of liangxiang hospital,fangshan district,Beijing from May 2015 to May 2019 were retrospectively analyzed.The control group underwent nasal endoscopic sinus surgery,while the research group underwent nasal endoscopic sinus surgery combined with low temperature plasma radiofrequency turbinate ablation.TNF-and il-6 levels and SNOT-20 and Lund-Kennedy scores before and after surgery were compared between the two groups.The clinical efficacy and VAS scores of the two groups were compared after 30 d operation.The incidence of postoperative complications was compared between the two groups.Results Compared with before surgery,SNOT-20,Lund-kennedy scores,serum TNF-α,IL-6 levels decreased in the two groups 30 days after surgery,and the study group was lower than the control group(P<0.05).VAS score of the study group was lower than that of the control group 30 days after surgery(P<0.05).After surgery for 30 days,the total effective rate of the study group(96.00%)was higher than the reference group(81.00%)(P<0.05).After surgery,the total incidence of complications in the study group(1.00%)was lower than that in the control group(7.00%)(P<0.05).Conclusion Low-frequency plasma turbinate radiofrequency ablation can reduce serum TNF-α and IL-6 levels in patients with chronic rhinosinusitis,improve patients'nasal cavity function,have significant curative effect,and without increasing the incidence of complications,is worthy of popularization and application.
