替格瑞洛治疗老年不稳定型心绞痛的效果及对凝血功能的影响

Clinical efficacy of ticagrelor on senile unstable angina pectoris and its effect on coagulation function

目的观察替格瑞洛治疗老年不稳定型心绞痛的临床疗效及对患者凝血功能的影响。方法选取收治的确诊为老年不稳定型心绞痛患者共100例纳入研究,遵循随机原则分为2组,其中对照组接受氯吡格雷+阿司匹林进行治疗,观察组采用替格瑞洛+阿司匹林进行治疗,比较2组治疗前后血小板反应指数(PRI)变化,以及PT、纤维蛋白原(FIB)、血小板最大聚集率(MPAR)等凝血功能指标变化,并比较口服治疗1年内患者心血管意外事件发生情况。结果与对照组比较,观察组患者治疗后PRI更低,差异有统计学意义(P<0.05);与对照组比较,观察组治疗后MPAR、FIB、PT均更低,差异有统计学意义(P<0.05);观察组心血管意外事件发生率为6%,低于对照组的28%,差异有统计学意义(P<0.05)。结论替格瑞洛治疗老年不稳定型心绞痛患者的临床疗效显著,对患者凝血功能状况影响不大,并可有效减低心血管事件发生的风险。

Objective o observe the clinical effect of ticagrelor in the treatment of senile unstable angina pectoris and its effect on coagulation function.Methods 100 elderly patients with unstable angina pectoris were selected and divided into two groups according to the principle of randomization. The control group was treated with clopidogrel and aspirin,while the observation group was treated with ticagrelor and aspirin. Platelet response index(PRI),changes in PT,fibrinogen(FIB),maximum platelet aggregation rate(MPAR) and other coagulation function indicators were detected and compared between the two groups before and after treatment,as well as the occurrence of cardiovascular accidents within one year after treatment.Results Compared with the control group,the PRI in the observation group was lower after treatment,the difference was statistically significant(P<0.05). Compared with the control group,the MPAR,FIB and PT in the observation group were lower after treatment,the difference was statistically significant(P<0.05). The incidence of cardiovascular accidents in the observation group was 6%,it was lower than that in the control group(28%). The difference was statistically significant(P<0.05).Conclusion The clinical efficacy of ticagrelor in elderly patients with unstable angina pectoris is significant,which has little effect on patients’ coagulation function and can effectively reduce the risk of cardiovascular events.
