磁共振胰胆管造影与64层螺旋CT诊断恶性梗阻性黄疸的比较

Comparison of magnetic resonance cholangiopancreatography and 64-slice spiral CT in the diagnosis of malignant obstructive jaundice

目的对比观察磁共振胰胆管造影(MRCP)、64层螺旋CT对恶性梗阻性黄疸的诊断效果。方法对2018年1月至2019年1月在我院治疗的88例恶性梗阻性黄疸患者进行研究,分别给予MRCP检查和64层螺旋CT检查,以病理结果为金标准,比较2组的诊断效果。结果以病理结果为金标准,2种诊断方法在梗阻水平定位的诊断上,MRCP符合率均为100%,CT为99%,比较差异无统计学意义(P>0.05);在肿瘤性病变诊断中,MRCP诊断符合率为94%,CT为91%,比较差异无统计学意义(P>0.05);梗阻原因诊断MRCP符合率为91%优于CT的78%,2组比较差异有统计学意义(P<0.05)。结论 MRCP、CT应用于恶性梗阻性黄疸的诊断中,MRCP在梗阻原因诊断比CT精确,但64层螺旋CT操作更简便,且无创无痛、费用低,因此,在临床诊断中,应根据患者的实际需求选择合适的诊断方法。

Objective To compare the diagnostic effects of magnetic resonance cholangiopancreatography(MRCP) and 64-slice spiral CT in malignant obstructive jaundice. Methods A total of 88 patients with malignant obstructive jaundice treated in our hospital from January 2018 to January 2019 were studied. MRCP examination and64-slice spiral CT examination were performed respectively. The pathological results were used as the gold standard to compare the diagnostic effects of the two groups. Results Taking the pathological result as the gold standard, the MRCP coincidence rate of the two diagnostic methods in the diagnosis of the location of obstruction is 100%, and the CT is 99%. There is no statistically significant difference(P>0.05);in the diagnosis of neoplastic lesions The coincidence rate of MRCP diagnosis was 94%, CT was 91%, the difference was not statistically significant(P>0.05);the MRCP coincidence rate of diagnosis of obstructive causes was 91% better than 78% of CT, the difference between the two groups(P<0.05). Conclusion MRCP and CT are more accurate than CT in the diagnosis of malignant obstructive jaundice,but 64-slice spiral CT is more convenient,painless and non-invasive. Therefore,in clinical diagnosis,the appropriate diagnosis method should be selected according to the actual needs of patients.
