摘要
目的探究孕早期糖化血红蛋白(HbA1c)与糖化血清蛋白(GSP)联合检测对妊娠期糖尿病的诊断价值。方法选取河南科技大学第一附属医院2018年1月-2019年4月期间60例确诊为妊娠期糖尿病的孕早期孕妇作为观察组,选取同期体检60例健康孕妇作为对照组,比较两组孕妇HbA1c、GSP、血糖水平。比较HbA1c、GSP与两者联合诊断妊娠期糖尿病的ROC曲线及诊断效能。结果观察组HbA1c、GSP、血糖水平均高于对照组(P<0.05);观察组HbA1c、GSP水平异常发生率分别为80.00%、85.00%高于对照组的6.67%、13.33%(P<0.05);由ROC曲线可知,利用孕早期HbA1c、GSP水平诊断妊娠期糖尿病的AUC为0.808、0.842,联合检查的AUC为0.946,HbA1c、GSP水平各自分析所得的AUC小于联合诊断(P<0.05)。结论在孕早期妊娠期糖尿病诊断中,HbA1c、GSP水平异常升高,HbA1c联合GSP检测可以提高诊断准确率。
Objective To investigate the diagnostic value of combined detection of glycosylated hemoglobin(HbA1 c) and glycosylated serum protein(GSP) in early pregnancy for gestational diabetes. Methods 60 pregnant women diagnosed with gestational diabetes mellitus from January,2018 to April,2019 were selected as observation group. 60 healthy pregnant women were selected as the control group. The HbA1 c,GSP and blood glucose levels of the two groups were compared. The ROC curve and diagnostic efficacy of HbA1 c,GSP and their combination in the diagnosis of gestational diabetes were compared. Results The HbA1 c,GSP and blood glucose levels in the observation group were higher than those in the control group(P<0.05). The abnormal incidence rates of HbA1 c and GSP in the observation group were 80.00% and 85.00%,respectively,which were higher than the control group 6.67% and 13.33%(P<0.05). According to the ROC curve,the AUC of gestational diabetes mellitus was 0.808 and0.842,and the AUC of the combined examination was 0.946. The AUC of HbA1 c and GSP levels were less than the combined diagnosis(P<0.05). Conclusion In the diagnosis of gestational diabetes in early pregnancy,HbA1 c and GSP levels are abnormally elevated,and HbA1 c combined with GSP detection can improve the diagnostic accuracy.
