摘要
目的:探讨T2DM患者中血清胃泌素-17(GAS-17)水平与外周自主神经病变(PAN)有无相关性.方法:选取287例T2DM患者为研究对象,电导分析仪SUDOSCAN测得的电化学皮肤导电值(ESC)作为检测外周自主神经病变程度的指标,根据双手、双足平均电导值(HESC、FESC)的差异分为三组:自主神经正常组114例(NPAN组),中度自主神经病变组92例(MPAN组)及重度自主神经病变组81例(SPAN组),比较三组患者血清指标差异.结果:与NPAN组相比,MPAN组患者的年龄、病程、收缩压较高,与NPAN、MPANF组相比,SPAN组患者的GAS-17较高(P<0.05).Spearman相关分析显示GAS-17水平与FESC、HESC呈负相关,与FASYM、HASYM呈正相关(P<0.05).多重线性回归分析结果显示,GAS-17是FESC、HESC的影响因素(P<0.05).结论:GAS-17可作为糖尿病重度外周自主神经病变的筛选方法之一.
Objective:To investigate the relationship between serum gastrin-17(GAS-17)level and peripheral autonomic neuropathy(PAN)in patients with type 2 diabetes mellitus(T2DM).Methods:A total of 287 patients with T2DM were enrolled in this study.The conductivity analyzer SUDOSCAN was used to measure the skin electrical conductivity(ESC)as an index for detecting peripheral autonomic function.All the patients were divided into three groups according to the sweat function:patients with normal peripheral autonomic nerve(NPAN group,n=114),patients with moderate peripheral autonomic neuropathy(MPAN group,n=92),and patients with severe peripheral autonomic neuropathy(SPAN group,n=81).The serum indexes were compared among the three groups.Results:The age,duration of diabetes and SBP in MPAN group were significantly higher than in NPAN group,while GAS-17 level was significantly higher in SPAN group.Spearman correlation analysis showed that GAS-17 level was negatively correlated with FESC and HESC,and positively correlated with FASYM and HASYM.Multivariate stepwise linear regression analysis showed that GAS-17 was the influencing factor for FESC and HESC.Conclusion:The serum GAS-17 can be used as the clinic screening methods for severe diabetic peripheral autonomic neuropathy.
