经侧裂手术联合亚低温对高血压脑出血患者神经功能及并发症的影响 被引量:1

Effect of Lateral Fissure Combined with Mild Hypothermia on Neurological Function and Complications in Patients with Hypertensive Intracerebral Hemorrhage

摘要 目的研究经侧裂手术联合亚低温对高血压脑出血患者神经功能及并发症的影响。方法选择2016年5月-2018年2月郑州市第三人民医院接受手术治疗的高血压脑出血患者60例,根据随机双盲法分为对照组和观察组,每组30例。对照组采用传统治疗方式,观察组采用经侧裂手术联合亚低温治疗。对比两组手术对神经功能、日常生活能力的影响,并统计其并发症发生率。结果术前,两组NIHSS、ADL评分对比,差异无统计学意义(P>0.05);观察组术后1周、1个月及3个月的ADL评分均比术前高,且观察组高于对照组,差异有统计学意义(P<0.05);观察组术后1周、1个月及3个月的NIHSS评分较术前低,且观察组低于对照组,差异有统计学意义(P<0.05);观察组术后并发症发生率(10.00%)比对照组(33.33%)低,差异有统计学意义(P<0.05)。结论经侧裂手术联合亚低温治疗高血压脑出血患者可获得良好的效果,且安全性高,可减少对机体神经功能及日常生活能力的影响,降低并发症发生情况,建议临床广泛应用。 Objective To study the effect of lateral fissure combined with mild hypothermia on neurological function and complications in patients with hypertensive intracerebral hemorrhage. Methods 60 patients with hypertensive intracerebral hemorrhage from May, 2016 to February, 2018 were randomly divided into control group(n=30)and observation group(n=30). The control group was treated with traditional treatment, while the observation group was treated with lateral fissure combined with mild hypothermia. The effects of operation on nerve function and ability of daily living were compared between the two groups, and the incidence of complications was counted. Results There was no significant difference in NIHSS and ADL score between the two groups before operation. The ADL scores of the observation group were higher than those of the control group 1 week, 1 month and 3 months after operation. The NIHSS scores of the observation group were significantly higher than those of the control group, and the NIHSS scores of the observation group were lower than those of the control group 1 week, 1 month and 3 months after operation. The difference was statistically significant(P<0.05). The incidence of postoperative complications in the observation group(10.00%)was lower than that in the control group(33.33%),and the difference was statistically significant(P<0.05). Conclusion Translateral fissure surgery combined with mild hypothermia can achieve good results in the treatment of hypertensive intracerebral hemorrhage. The safety is high, which can reduce the influence on the nerve function and the ability of daily living, reduce the occurrence of complications. So it should be widely used in clinic.
