摘要
目的探讨红细胞分布宽度(RDW)与尿毒症患者髂动脉粥样硬化的关系。方法选取连山县人民医院2016年4月-2019年4月尿毒症患者156例,检查发现髂动脉粥样硬化的患者78例归为观察组;无髂动脉粥样硬化患者78例归为对照组。比较两组患者外周血中各项指标的差异性。运用二元logistic回归分析髂动脉粥样硬化的危险因素。绘制ROC曲线,计算AUC。结果 (1)观察组与对照组中RDW、血小板计数、尿酸、甲状旁腺激素差异有统计学意义(P<0.05)。(2)将RDW、血小板计数、尿酸、甲状旁腺激素进行二元logistic回归分析后进行联合预测,其AUC值为0.553,特异度为0.592,敏感度为0.603,P=0.463,95%CI (1.653~2.046)。(3)将RDW进行二元logistic回归分析并绘制ROC曲线显示RDW与尿毒症髂动脉粥样硬化具相关性,其AUC值为0.902,特异度为0.962,敏感度0.927,95%CI(0.848~0.957),P=0.001。结论尿毒症患者髂动脉粥样硬化与RDW相关,对RDW动态监测能有效预测尿毒症患者的髂动脉粥样硬化。
Objective To investigate the relationship between red blood cell distribution width(RDW) and atherosclerosis in uremic patients. Methods A total of 156 patients with uremia from April, 2016 to April, 2019 in the hospital were enrolled. 78 patients with atherosclerosis were classified as observation group, and 78 patients without atherosclerosis were classified as control group. The differences in various indexes in peripheral blood of the two groups were compared. Binary logistic regression was used to analyze the risk factors of atherosclerosis. A work characteristic correlation curve(ROC curve) was drawn, and the area under the curve(AUC) was caculated. Results(1)The difference of red blood cell distribution width(RDW), platelet count, uric acid and parathyroid hormone between the observation group and the control group was statistically significant(P<0.05).(2)The red blood cell distribution width(RDW), platelet count, uric acid and parathyroid hormone were analyzed by binary logistic regression analysis. The area under the curve(AUC) was 0.553, the specificity was 0.592, and the sensitivity was 0.603, P=0.463, 95%CI(1.653-2.046).(3)RDW was performed by binary logistic regression analysis and ROC curve was drawn to show the correlation between cell distribution width(RDW) and uremic atherosclerosis. The area under the curve was 0.902, the specificity was 0.962, and the sensitivity was 0.927, 95%CI(0.848-0.957), P=0.001. Conclusion The atherosclerosis of uremic patients is associated with the distribution of red blood cells(RDW). Dynamic monitoring of red blood cell width(RDW) can effectively predict radial atherosclerosis in uremic patients.
