摘要 目的探讨对比剂不同注射部位选择对(并有)大型房间隔缺损的先天性心脏病CT图像质量影响的应用价值。方法收集70例(并有)大型房间隔缺损的先天性心脏病患儿,按照注射部位分为两组,A组为上腔静脉注入,B组为下腔静脉注入,对比剂均使用300mgI/ml碘海醇,使用双筒高压注射器,对比剂注射完追注3-5ml生理盐水冲管。分别测量四腔室右心房、左心房的CT值、噪声及信噪比,并进行主观评分。结果A、B两组右心房的CT值、噪声、SNR差异有统计学意义。A、B组检查成功率分别为100%(35/35)、85.7%(30/35)。A、B两组的心内及大血管主观评分差异有无统计学意义。A、B两组的诊断准确性分别为94.9%(75/79)、93.4%(85/91),差异无统计学意义。结论(并有)大型房间隔缺损病人检查前选用上腔静脉注入对比剂能提高检查的成功率。 Objective Purpose To explore the effect of different injection site selection on CT image quality of congenital heart disease with large atrial septal defect. Methods A total of 70 cases of congenital heart disease with large atrial septal defect were collected. The injection site was divided into two groups, group A was the superior vena cava injection, and group B was the inferior vena cava injection. Comparison agents were all used with 300 mgI/ml iohexol. The contrast agent was injected with 3-5 ml normal saline rinsing tube. CT values, noise and signal-to-noise ratio of the right and left atria of the four Chambers were measured and scored subjectively. Results The CT value, noise and SNR of the right atrium of group A and group B had statistical significance. The success rate of group A and group B was 100%(35/35) and 85.7%(30/35) respectively. There was no statistically significant difference in subjective score of intracardiac and macrovascular between groups A and B. The diagnostic accuracy of group A and group B was 94.9%(75/79) and 93.4%(85/91) respectively, and the difference was not statistically significant. Conclusion Contrast agent injection of superior vena cava before examination can improve the success rate of large atrial septal defect patients.
