单纯后路治疗胸腰椎结核的效果及并发症观察

Observation on Effects and Complications of Simple Posterior Approach in the Treatment of Thoracolumbar Tuberculosis

目的研究单纯后路手术方式用于治疗胸腰椎结核的临床效果,并观察术后并发症发生情况。方法将本院2015年12月-2019年4月收治的72例胸腰椎结核患者使用随机数字表法分为观察组和对照组,各36例。观察组使用单纯后路手术方式进行治疗,对照组使用单纯前路手术方式进行治疗,记录两组手术情况(术中出血量和手术耗时);在出院时统计两组的平均住院时间和住院费用,并评价两组的治疗效果和并发症发生率。结果 (1)在治疗效果方面,观察组为94.44%,高于对照组的86.11%,两组比较差异无统计学意义(P>0.05);(2)观察组的术中出血量、手术耗时、住院时间和住院费用均低于对照组,两组比较差异无统计学意义(P<0.05);(3)两组术后并发症比较:观察组为11.11%,低于对照组的19.44%,两组比较差异无统计学意义(P>0.05)。结论单纯后路方式能有效降低胸腰椎结核患者手术耗时和术中出血量,并能降低住院费用,缩短住院时间,具有较好的治疗效果。

Objective To study the clinical effect of simple posterior approach in the treatment of thoracolumbar tuberculosis and to observe the occurrence of postoperative complications. Methods A total of 72 patients with thoracolumbar tuberculosis admitted to the hospital from December 2015 to April 2019 were randomly divided into observation group and control group according to the random number table method, with 36 cases in each group. Observation group was treated with simple posterior approach, and control group was treated with simple anterior approach. The operation status(intraoperative blood loss and operative time) was recorded in the two groups. The average hospital stay and hospitalization cost were counted in the two groups at discharge, and the treatment effects and incidence rate of complications were evaluated in the two groups. Results(1) The treatment effect in observation group was higher than that in control group(94.44%vs86.11%)(P>0.05).(2) The intraoperative blood loss, operative time, hospital stay and hospitalization cost in observation group were lower than those in control group(P<0.05).(3) The incidence rate of postoperative complications in observation group was lower than thatin control group(11.11%vs 19.44%)(P>0.05). Conclusion Simple posterior approach can effectively reduce the operative time and intraoperative blood loss inpatients with thoracolumbar tuberculosis, and it helps toreduce the hospitalization cost and shorten the hospital stay, and it has better treatment effects.
