温馨助产服务模式联合产时体位管理对初产妇分娩产程及分娩质量的影响

The influence of warm midwifery service mode combined with postural management on the delivery process and delivery quality of the primiparas

目的研究初产妇分娩采用温馨助产服务模式联合产时体位管理的效果。方法选取2016年1月至2019年2月本院妇产科接收的110名初产妇,按照随机数字表法分为观察组(温馨助产服务模式联合产时体位管理)与对照组(常规指导),各55名。比较两组的产程、出血量、观察指标、分娩结局及分娩方式选择。结果与对照组比较,观察组第一、第二、第三产程及总产程时间均较短,产后2 h出血量少,疼痛阈值、焦虑与抑郁评分低,新生儿窒息、胎儿窘迫、产后出血及酸中毒发生率低,自然分娩率高,剖宫产率低,差异均具有统计学意义(P<0.05)。结论温馨助产服务模式联合产时体位管理能改善初产妇的分娩结局,缩短产程并缓解疼痛,提高分娩质量。

Objective To study the effect of combined midwifery position management during the delivery of primiparas with warm midwifery service model. Methods From January 2016 to February 2019, 110 primiparas received by our Obstetrics and Gynecology Department were selected and divided into observation group(comfortable midwifery service model combined with postnatal position management) and control group(routine guidance) according to the random number table method), with 55 cases in each group. The two groups were compared in terms of labor, bleeding, observations, delivery outcomes, and choice of delivery methods. Results Compared with the control group, the first, second, third, and total births were shorter in the observation group, with less bleeding at 2 h postpartum, lower pain threshold, anxiety and depression scores, neonatal asphyxia, fetal distress. The incidence of postpartum hemorrhage and acidosis was low, the rate of natural delivery was high, and the rate of cesarean section was low. The difference was statistically significant(P<0.05). Conclusion The warm midwifery service mode combined with postpartum position management can improve the delivery outcomes of first-time women, shorten the labor process and relieve pain, and improve the quality of delivery.
