摘要
目的:探讨妊娠期胎儿体表畸形应用四维彩超诊断的临床价值。方法:以2017年11月~2018年11月在本院产检的278例孕妇作为研究对象(所有病例均经产后证实),对278例孕妇的病例资料进行回顾性分析。所有孕妇均分别采用四维彩超和二维彩超进行检查,比较两种检查方法胎儿体表畸形的诊断准确情况。结果:二维彩超诊断准确率为60.00%(15/25),漏诊率为40.00%(10/25);四维彩超诊断准确率为92.00%(23/25),漏诊率为8.00%(2/25),四维彩超的诊断准确率明显高于二维彩超,漏诊率明显低于二维彩超,差异具有统计学意义(P<0.05)。四维彩超和二维彩超在前腹壁、颜面部、四肢、神经系统等部位检出畸形比例相比,差异均无统计学意义(P>0.05)。结论:在产前胎儿畸形诊断中,四维彩超有较好的应用价值,它的图像能直观、立体,胎儿体表畸形诊断准确率高。
Objective:To explore the clinical value of applying four-dimensional color Doppler ultrasound in the diagnosis of fetal surface abnormalities during pregnancy.Methods:Taking 278 pregnant women in our hospital from November 2017 to November 2018 as the research object(all cases were confirmed postpartum),retrospectively analyzed the case data of 278 pregnant women.All pregnant women were examined by four-dimensional color Doppler ultrasound and twodimensional color Doppler ultrasound,and the accuracy of the diagnosis of fetal surface malformations was compared between the two methods.Results:The diagnostic accuracy rate of 2D color Doppler ultrasound was 60.00%(15/25),the rate of missed diagnosis was 40.00%(10/25);the accuracy rate of 4D color Doppler ultrasound diagnosis was 92.00%(23/25),and the rate of missed diagnosis was 8.00%(2/25)The diagnostic accuracy rate of four-dimensional color Doppler ultrasound is significantly higher than that of two-dimensional color Doppler ultrasound,and the rate of missed diagnosis is significantly lower than that of two-dimensional color Doppler ultrasound.The difference is statistically significant(P<0.05).There was no statistically significant difference in the proportion of abnormalities detected between the four-dimensional color Doppler ultrasound and the two-dimensional color Doppler ultrasound in the anterior abdominal wall,face,limbs,and nervous system(P>0.05).Conclusion:In the diagnosis of prenatal fetal malformations,four-dimensional color Doppler ultrasound has good application value,its images can be intuitive and stereoscopic,and the accuracy of fetal surface malformation diagnosis is high.
