悬吊式腹腔镜胆囊切除术对胆结石患者免疫功能及胃肠功能的影响

Effects of suspended laparoscopic cholecystectomy on immunologic function and gastrointestinal function in patients with gallstone

目的探讨悬吊式腹腔镜胆囊切除术对胆结石患者免疫功能及胃肠功能的影响。方法选取2014年4月至2019年5月我院收治的胆结石患者130例,按手术方案的不同将其分为观察组和对照组,每组65例。对照组行常规腹腔镜胆囊切除术,观察组行悬吊式腹腔镜胆囊切除术。比较两组的治疗效果。结果观察组术后拔管时间、术后肛门排气时间、术后进食时间均短于对照组,差异有统计学意义(P<0.05)。术后1 d,两组的CD3^+、CD4^+均降低,但观察组高于对照组,CD8^+均升高,但观察组低于对照组,差异有统计学意义(P<0.05)。观察组肠黏膜损伤程度轻于对照组,差异有统计学意义(P<0.05)。观察组并发症总发生率为1.54%(1/65),与对照组的3.08%(2/65)比较,差异没有统计学意义(P>0.05)。结论悬吊式腹腔镜胆囊切除术治疗胆结石的效果确切,能进一步降低对免疫功能及胃肠功能的影响,且并发症少,安全性高。

Objective To investigate the effects of suspended laparoscopic cholecystectomy on immunologic function and gastrointestinal function in patients with gallstone.Methods From April 2014 to May 2019,130 patients with gallstone admitted in our hospital were selected and divided into observation group and control group according to different operation plans,with 65 cases in each group.The control group underwent routine laparoscopic cholecystectomy,and the observation group underwent suspended laparoscopic cholecystectomy.The therapeutic effects of the two groups were compared.Results The postoperative extubation time,anal exhaust time and feeding time of the observation group were shorter than those of the control group,and the differences were statistically significant(P<0.05).At 1 d after operation,CD3^+and CD4^+in both groups decreased,but those in the observation group were higher than the control group,and CD8^+increased,but that in the observation group was lower than the control group,the differences were statistically significant(P<0.05).The injury degree of intestinal mucosa in the observation group was lighter than that in the control group,and the difference was statistically significant(P<0.05).The total incidence of complications in the observation group was 1.54%(1/65),which was not statistically significant compared with 3.08%(2/65)in the control group(P>0.05).Conclusion Suspended laparoscopic cholecystectomy has a definite effect in the treatment of gallstone,which can further reduce the influences on immunologic function and gastrointestinal function,with fewer complications and high safety.
