宫腔镜治疗慢性宫颈炎患者的临床价值

Clinical value of hysteroscopy in the treatment of chronic cervicitis

目的研究宫腔镜治疗慢性宫颈炎患者的临床价值。方法选择2015年9月至2018年9月西安电子科技大学医院和西安医学院第一附属医院接收并诊治的118例慢性宫颈炎患者为研究对象,采用随机数字法将其分为参照组和宫腔镜组,每组59例。参照组采用聚焦超声激光方法进行治疗,宫腔镜组采用宫腔镜手术进行治疗。比较两组患者的治疗效果。结果宫腔镜组治疗1、2个月后宫颈上皮覆盖率均明显高于参照组(P<0.05)。宫腔镜组脓性黏液白带消失时间、带血白带消失时间、下腹或腰骶疼痛消失时间均明显短于参照组(P<0.05)。治疗1个月后,宫腔镜组临床治疗总有效率明显高于参照组(P<0.05)。治疗1个月后,两组的NRS评分均明显降低,且宫腔镜组低于参照组(P<0.05)。治疗1个月后,两组的TNF-α、IFN-γ、IL-8水平均降低,且宫腔镜组明显低于参照组(P<0.05)。宫腔镜组治疗1个月后并发症总发生率明显低于参照组(P<0.05)。结论宫腔镜治疗慢性宫颈炎患者的临床价值显著,值得推广临床应用。

Objective To study the clinical value of hysteroscopy in the treatment of chronic cervicitis.Methods A total of 118 patients with chronic cervicitis received and treated by Xidian university hospital and the first affiliated hospital of Xi’an medical university from September 2015 to September 2018 were selected as the study objects.The patients were divided into reference group and hysteroscopy group by random number method,with 59 cases in each group.The reference group was treated by focused ultrasound laser,and the hysteroscopy group was treated by hysteroscopic surgery.The therapeutic effects of two groups were compared.Results At 1 and 2 months after treatment,the coverage rates of cervical epithelium in the hysteroscopy group were significantly higher than those in the reference group(P<0.05).The disappearance time of purulent mucinous leucorrhea,leucorrhea with blood,and pain of lower abdomen or lumbosacral in the hysteroscopy group were significantly shorter than those in the reference group(P<0.05).After 1 month of treatment,the total effective rate of clinical treatment of the hysteroscopy group was significantly higher than that of the reference group(P<0.05).After 1 month of treatment,the NRS score of both groups decreased significantly,and that in the hysteroscopy group was lower than the reference group(P<0.05).After 1 month of treatment,the levels of TNF-α,IFN-γand IL-8 in the two groups decreased,and those in the hysteroscopy group were significantly lower than the reference group(P<0.05).After 1 month of treatment,the total incidence of complications in the hysteroscopy group was significantly lower than that in the reference group(P<0.05).Conclusion The clinical value of hysteroscopy in the treatment of chronic cervicitis is significant,which is worthy of clinical promotion and application.
