围手术期个体化优质护理在行腹腔镜胆囊切除术的胆结石伴糖尿病患者中的应用价值

Application value of individualized high quality nursing in perioperative period of patients with cholelithiasis and diabetes mellitus undergoing laparoscopic cholecystectomy

目的分析围手术期个体化优质护理在行腹腔镜胆囊切除术的胆结石伴糖尿病患者中的应用效果。方法将2014年1月至2019年1月在我院接受腹腔镜胆囊切除术的150例胆结石伴糖尿病患者随机分为对照组与观察组,各75例。对照组围手术期采用常规护理,观察组围手术期采用个体化优质护理。比较两组患者的护理效果。结果观察组患者的术后排气时间、下床活动时间、切口愈合时间、住院时间均短于对照组(P<0.05)。手术当天、术后1 d,观察组患者的FPG、2 h PG水平均低于对照组(P<0.05)。术后,两组患者的ATCH、AD水平均升高,SAS、SDS评分均降低,且观察组均低于对照组(P<0.05)。观察组术后PQRS评分低于对照组(P<0.05)。结论围手术期个体化优质护理可以促进行腹腔镜胆囊切除术的胆结石伴糖尿病患者的术后恢复,稳定血糖水平,减轻手术应激反应,促进术后心理及生理恢复,值得临床推广应用。

Objective To analyze the application effect of individualized high quality nursing in perioperative period of patients with cholelithiasis and diabetes mellitus undergoing laparoscopic cholecystectomy.Methods From January 2014 to January 2019,150 patients with cholelithiasis and diabetes mellitus who underwent laparoscopic cholecystectomy in our hospital were randomly divided into control group and observation group,with 75 cases in each group.The control group received routine nursing in perioperative period,and the observation group received individualized highquality nursing in perioperative period.The nursing effects were compared between the two groups.Results The postoperative exhaust time,out of bed activity time,incision healing time and hospitalization time in the observation group were shorter than those in the control group(P<0.05).At the day of operation and one day after operation,the levels of FPG and 2 h PG in the observation group were lower than those in the control group(P<0.05).After operation,the levels of ATCH and AD in the two groups increased,the scores of SAS and SDS decreased,and those in the observation group were lower than the control group(P<0.05).The PQRS scores after operation of the observation group was lower than that of the control group(P<0.05).Conclusion The individualized high quality nursing in perioperative period of patients with cholelithiasis and diabetes mellitus undergoing laparoscopic cholecystectomy can promote postoperative recovery,stabilize blood glucose level,reduce stress response,and promote psychological and physiological recovery,which is worthy of clinical promotion and application.
