摘要
目的了解北京男男性行为者(MSM)1型艾滋病病毒(HIV-1)基因多样性情况及影响因素。方法对北京HIV阳性MSM采用横断面调查,通过建立的基于Illumina平台的二代测序技术(NGS),综合两个部分pol基因区片段和一个env基因区片段的结果进行亚型和系统进化分析。结果获得NGS结果的376例样本中,单一亚型样本313例(占83.2%),重组率和共感染率分别为8.2%(31例)和8.5%(32例)。313例单一亚型样本中CRF01_AE亚型188例(60.1%),CRF07_BC亚型86例(27.5%),B亚型32例(10.2%),C亚型3例(1.0%),CRF55_01B亚型4例(1.3%)。376例样本中,亚型内重组率为1.9%(7例),亚型间重组率为6.4%(24例);亚型内共感染率为1.3%(5例),亚型间共感染率为7.2%(27例)。多因素Logistic回归分析结果显示,与HIV-1基因多样性相关的影响因素是无工作(P=0.03),过去3个月使用新型毒品(P=0.03),和通过互联网寻找男性性伴侣的方式(P=0.01)。结论北京MSM的HIV感染者中有较高的HIV-1基因多样性,反映了HIV阳性MSM复杂的高危行为。
Objective To investigate the HIV-1 gene diversity and influencing factors in Beijing MSM population. Methods A cross-sectional study of HIV-positive MSM population in Beijing MP3 project was conducted to establish an experimental and research method based on the Illumina platform’s next generation sequencing technology, while two pol gene regions and one env gene region were analyzed. Results Among the 376 samples tested by NGS, the rate of single subtype was 83.2%(313/376) and the main subtypes were CRF01_AE(60.1%, 188/313), CRF07_BC(27.5%, 86/313), subtype B(10.2%, 32/313), subtype C(1.0%, 3/313) and CRF55_01B(1.3%, 4/311), while the recombination and super-infection accounted for 8.24%(31/376) and 8.51%(32/376) respectively. The intrasubtype recombination rate was 1.9%(7/376), the intersubtype recombination rate was 7.2%(27/376);the intrasubtype superinfection rate was 1.3%(5/376) and the intersubtype superinfecton rate was 7.2%(27/376). Analysis of relevant influencing factors showed that the risk factors associated with HIV diversity were working status(P=0.03), whether new drugs were used in the past three months(P=0.03), and the way to find male sexual partners(P=0.01). Conclusion We establish experimental and research methods for detecting HIV diversity based on NGS sequencing technology;HIV diversity is associated with working status, the use of new drugs and the way to find male sexual partners through the Internet.
