摘要
目的分析空心加压螺钉(CCS)、动力髋螺钉螺旋刀片(DHS-B)治疗股骨颈骨折的近远期疗效差异。方法回顾性分析2017年12月~2018年12月我院收治的80例股骨颈骨折患者的临床资料,根据治疗方式的不同分为对照组(n=40)和观察组(n=40),对照组采用CCS治疗,观察组采用DHS-B治疗,比较两组手术时间、术中出血量、住院时间、髋关节功能(Harris)评分及并发症发生率。结果观察组手术、住院时间均短于对照组,术中出血量多于对照组(P<0.05);两组术后1个月Harris评分均无显著差异(P>0.05),观察组术后12个月Harris评分显著高于对照组(P<0.05);观察组并发症发生率为7.50%显著低于对照组的32.50%(P<0.05)。结论 CCS、DHS-B治疗股骨颈骨折的近期疗效相当,CCS手术及住院时间较短,术中出血量少,DHS-B远期疗效较好,且安全性较高。
Objective To analyze the short-term efficacy and long-term efficacy of cannulated compression screws(CCS) and dynamic hip screw blades(DHS-B) in the treatment of femoral neck fractures. Methods The clinical data of 80 patients with femoral neck fractures admitted to our hospital from December 2017 to December 2018 were retrospectively analyzed. According to the different treatment methods, they were divided into control group(n=40) and observation group(n=40). Control group was treated with CCS, and observation group was treated with DHS-B. The operative time, intraoperative blood loss, hospital stay, hip function score(Harris) score and incidence rate of complications were compared between the two groups. Results The operative time and hospital stay in observation group were shorter than those in control group, and the intraoperative blood loss was more than that in control group(P<0.05). There was no significant difference in the Harris score at 1 month after operation between the two groups(P>0.05), and the Harris score at 12 months after operation in observation group was significantly higher than that in control group(P<0.05). The incidence rate of complications in observation group was significantly lower than that in control group(7.50% vs 32.50%)(P<0.05). Conclusion CCS and DHS-B have similar short-term effects in the treatment of femoral neck fractures, and CCS has shorter operative time and hospital stay and less intraoperative blood loss, and DHS-B has better long-term efficacy and higher safety.
