儿童支原体肺炎支气管镜下气道黏膜改变与中医证型、舌象相关性研究

Correlation study between changes of airway mucosa of mycoplasmal pneumonia in children under bronchoscopy and TCM syndrome types and manifestations of tongue

目的探讨儿童支原体肺炎(MPP)支气管镜下气道黏膜改变与中医证型、舌象的相关性。方法选取2016年5月至2018年1月就诊于天津中医药大学第一附属医院儿科住院部的MPP患儿76例,观察患儿支气管镜下黏膜微观病理改变以及患儿中医证型、舌象的特点,采用SPSS 19.0软件分析数据。结果MPP患儿镜下气道黏膜表现在各中医证型间分布的差异有统计学意义(P<0.05),Cramer’s V系数为0.397,P<0.01,说明MPP的中医证型与镜下黏膜表现呈中等相关性。舌质分布经Fisher精确概率检验法,Fisher值=2.446,P=0.319>0.05,说明MPP患儿舌质分布与镜下气道黏膜表现之间无相关性。MPP患儿舌苔与镜下黏膜表现之间的差异有统计学意义(P<0.05),同时Cramer’s V系数为0.428,P<0.01,说明MPP舌苔与镜下气道黏膜表现在一定程度上具有相关性。结论儿童MPP的中医证型、舌象与其支气管镜下气道黏膜征象的分型具有相关性。

Objective To discuss the correlation between airway mucosal changes of mycoplasmal pneumonia(MPP)in children under bronchoscopy and TCM syndrome types and manifestations of tongue.Methods Seventy-six cases of MPP consulted in pediatric inpatient department of the First Affiliated Hospital of Tianjin University of Chinese Medicine from May 2016 to January 2018 were chosen to observe the microscopic pathologic changes of mucosa under bronchoscope,and the characteristics of TCM clinical syndrome types and manifestations of tongue.SPSS 19.0 software was used for data analysis.Results There was a significant difference in the distribution of mucosal manifestations under bronchoscopy among TCM syndrome types in children with MPP(P<0.05),the V coefficient of Cramer was 0.397,P<0.01,which indicated that the TCM syndrome types of MPP had a moderate correlation with the mucosal manifestation under microscope.The distribution of tongue texture was tested by Fisher exact probability method,Fisher value=2.446,P=0.319>0.05,which indicated that there was no correlation between the distribution of tongue texture and the airway mucosal manifestations under microscope in children with MPP.There was a significant difference between the tongue coating and the mucosal manifestations under microscope in children with MPP(P<0.05).Meanwhile,the V coefficient of Cramer was 0.428,P<0.01,which indicated that the tongue coating of MPP was related to the airway mucosal manifestations under microscope to a certain extent.Conclusion The TCM syndrome types and tongue texture of children with MPP are correlated with the classification of airway mucosal signs under bronchoscope.
