庄礼兴教授治疗中风病用药规律数据挖掘研究

Analysis of Zhuang Lixing’s Prescription Rules For Stroke Based on Data Mining

目的:探索靳三针传承人庄礼兴教授治疗中风病的用药规律。方法:研究对象为庄教授近10年所治疗的中风病例,提取其处方药物,录入“中医传承辅助平台(V2.5)”软件并建立处方数据库,使用关联规则、复杂系统熵聚类等数据挖掘方法,分析其治疗中风的用药规律。结果:通过对筛选出的261首有效处方,涵盖药物123味,分析归纳处方中药物的使用频次前三位为炙甘草、当归、鸡血藤,得出4个核心药对“白芍+炙甘草”“鸡血藤+当归”“鸡血藤+五指毛桃”“地龙+僵蚕”与8首新方。结论:庄礼兴教授认为中风急性期应清热熄风化痰,恢复期注重活血,后遗症期注重补气。其还重用活血类药物,在补阳还五汤的基础上用南芪代替北芪,体现其“立足于补气血,补气而不壅滞”的原则,为临床治疗中风病提供了理论基础。

Objective:To explore the medication law of professor Zhuang Lixing in the treatment of stroke.Methods:The subjects of this study were stroke cases treated by professor Zhuang from 2009to 2017,whose prescription drugs were input to The Traditional Chinese Medicine Inheritance Support System,and established the prescription database.Association rules,complex system entropy clustering were used to analyze the medication law.Results:Base on the analysis of 261effective prescriptions,the association rules among the herbs and the frequency of each herb were determined.4core drug pairs and 8new prescriptions were discovered.The top 3most frequently used herbs were Broiled Liquorice,Angelica,Spatholobi Caulis.The 4core drug pairs were“Radix Paeoniae Alba+Broiled liquorice”“Angelica+Spatholobi Caulis”“Spatholobi Caulis+Fici Hirtae Radix”“Pheretima+Bombyx Batryticatus”.Conclusion:Professor Zhuang Lixing believes that we should clear heat,extinguish wind and reduce phlegm in the acute stage,circulate blood in the convalescence stage while replenish qi in the sequelae stage.In the course of treatment,blood-activating herbs are often used,what’more,Radix Astraga.Li is often replaced by Radix Fici Hirtae.It reflects the principle of basing on qi and blood while replenishing qi without stagnation.
