危重型新型冠状病毒肺炎成功救治经验 被引量:5

Experiences from the managements of a recovered critically ill patient with corona virus disease 2019

摘要 武汉大学人民医院重症医学科(ICU)2020年1月6日收治1例新型冠状病毒肺炎(简称新冠肺炎)危重型患者,经过30d的抢救治疗,患者转危为安,最终出院.从治疗结果来看,无论临床治疗反应、体温变化、还是实验室检查以及影像学,均表明本中心的整体治疗方案有效.因此,对于新冠肺炎危重型患者,尽早气管插管行有创机械通气,改善机体氧合,纠正顽固性低氧血症是救治成功的关键.恰当时机使用甲泼尼龙,合理应用抗菌药物,加强营养治疗等整体措施,是救治成功的保障.经检索,本例患者为新冠肺炎疫情发生以来全球第1例接受有创机械通气救治成功的病例. A critically ill patient with corona virus disease 2019(COVID-19)was admitted to the Department of Intensive Care Unit(ICU),Renmin Hospital of Wuhan University on January 6th,2020.After 30 days staying at ICU,the patient became stable and was discharged successfully.From the view of outcomes,whether clinical treatment reaction,or changes of temperature,results of laboratory and imaging examination,the overall treatment plan for this patient of our center is effective.So,in the therapeutic strategies for the critically ill type of COVID-19,early intubation and invasive mechanical ventilation to improve oxygenation and refractory hypoxemia is the key point to a good outcome.At the same time,proper methylprednisolone use,empiric broad-spectrum antibiotic and adequate nutrition therapy are also the important main parts of the combination therapy strategy.It has been confirmed by searching PubMed and Google Scholar that this case was the first successful critical patient who received early intubation and invasive mechanical ventilation since the outbreak of COVID-19.
