长沙市首例新型冠状病毒肺炎重型病例救治体会

Treatment experience of the first severe case of corona virus disease 2019 in Changsha City

长沙市第一医院公共卫生救治中心收治了长沙市首例新型冠状病毒肺炎(简称新冠肺炎)患者,及时应用抗病毒药物及抗炎等综合治疗,病情仍持续发展,合并呼吸衰竭,一度考虑拟气管插管有创呼吸机支持,但通过及时床旁重症超声监测,考虑呼吸衰竭原因为非心源性肺水肿,遂给予高流量吸氧装置加强氧疗、严格容量管理、限制补液及适当脱水减轻肺水肿,并加强免疫支持等综合治疗后,患者病情得到明显改善,经团队讨论后得出相关治疗经验,为后续新冠肺炎的救治,尤其是重型病例的救治提供一定参考依据.

The first patient of corona virus disease 2019(COVID-19)in Changsha City,Hunan Province had been timely given antiviral drugs,anti-inflammation and other comprehensive treatment,while the disease was still progressing and combined with respiratory failure,and obviously aggravated after activity.At one time,it was considered that endotracheal intubation should be applied for invasive ventilator support.However,the cause of respiratory failure was considered to be non cardiogenic pulmonary edema through timely bedside critical care ultrasound monitoring.Therefore,high flow oxygen inhalation device was given to strengthen oxygen therapy,strict volume management,limit fluid replenishment and appropriate dehydration were carried out to reduce pulmonary edema,and after immune support and other comprehensive treatment,the patient's condition improved significantly After discussion by the team,relevant treatment experience was obtained,some references were provided for the treatment of COVID-19,especially for the severe cases.
