摘要 目的探究气相色谱-质谱联用法(GC/MS)检测慢性肾脏病患者血清及粪便中短链脂肪酸(SCFAs)含量的可行性,并筛选特定SCFAs应用于监测肾衰进程及评估其作为诊断慢性肾脏病参考指标的可能性。方法收集南方医科大学南方医院健康成年及慢性肾脏病患者的血清及粪便,利用GC/MS检测上述标本中SCFAs(乙酸、丙酸、丁酸)的含量。结果所有标本均检测出乙酸、丙酸、丁酸。正常对照组(健康成年)血清和粪便中乙酸最高,其次为丙酸,丁酸含量最低;慢性肾脏病组血清和粪便中三种SCFAs的相对丰度与生理状态相同,但各个酸的含量较生理状态都有所减少。粪便中乙酸含量减少最为显著,血清中乙酸与丙酸减少较明显。无论在血清或是粪便中,慢性肾脏病组丁酸含量较正常对照组明显减少。结论GC/MS可快速检测慢性肾脏病患者血清及粪便中SCFAs的含量,丁酸可能应用于监测肾衰进程及成为慢性肾脏病辅助诊断的参考指标。 Objective To explore the feasibility of gas chromatography-mass spectrometry(GC/MS)to detect the content of short chain fatty acids in serum and feces of patients with chronic kidney disease(CKD),and to screen specific short chain fatty acids to monitor the process of renal failure and evaluate the possibility of them as a reference index for the diagnosis of CKD.Methods Serum and feces were collected from healthy adults and patients with CKD in the Southern Hospital of Southern Medical University,and determined by GC/MS.Results Acetic acid,propionic acid and butyric acid were detected in all samples.Acetic acid was the highest in serum and feces,followed by propionic acid,and butyric acid was the lowest in healthy controls.The relative abundances of the three short-chain fatty acids in serum and feces in patients with CKD were the same as those in controls,but the content of each acid was decreased.The content of acetic acid in feces decreased most,while the reduced levels of acetic acid and propionic acid were more obvious than those of butyric acid in serum.Conclusion GC/MS can rapidly detect the content of short-chain fatty acids in serum and feces of patients with CKD,and may monitor the progression of renal failure and become a reference index for the auxiliary diagnosis of CKD.
