尿NGAL与慢性肾脏病进展的相关分析

A meta-analysis on the relationship between urinary NGAL and chronic kidney diseases

目的探讨尿中性粒细胞明胶酶相关脂质运载蛋白(NGAL)与慢性肾脏病(CKD)进展的相关性。方法计算机检索外文数据库PubMed、 Embase及中文数据库万方、中国知网(CNKI)、中国维普网2018年10月前发表的关于尿NGAL与CKD相关性的研究。数据分析采用RevMan 5.3软件。结果共计纳入8篇文献,总研究样本量8 169例。结果显示,尿NGAL水平,CKD组高于对照组[SMD 0.46,95%CI(0.20~0.72)],差异有统计学意义(P<0.05);进一步亚组分析,与低NGAL水平组相比,高NGAL水平组患者eGFR降低[SMD-0.54,95%CI(-0.60^-0.48)],差异有统计学意义(P<0.05);蛋白尿增高[MD 2.01,95%CI(1.07~2.96)],差异有统计学意义(P<0.05);血压差异无统计学意义(P>0.05);与CKD非进展组相比,CKD进展组NGAL水平高[MD 180.72,95%CI(116.8~244.65)],差异有统计学意义(P<0.05)。结论尿NGAL水平与慢性肾脏病进展相关,应及时监测尿NGAL水平,早期防治CKD进展。

Objective To research the correlation of eutrophil gelatinase associated lipocalin NGAL)and the progression of chronic kidney disease(CKD).Methods Literatures related to NGAL and CKD had been searched and collected from databases,including PubMed,Embase,CNKI,Wanfang,China web.Meta-analysis was obtained using Review Manager Software 5.3.Results A total of 8 articles and 8169 cases were included in the study.NGAL were obviously higher than the control group(SMD 0.46,95%CI:0.20~0.72)(P<0.05).Subgroup analysis:The levels of 24-hour proteinuria(MD 2.01,95%CI:1.07~2.96)(P<0.05),eGFR(SMD-0.54,95%CI-0.60-0.48)(P<0.05)in higher NGAL level group were significantly difference.There was no different significance in the levels of BP(P>0.05).The levels of urine NGAL in CKD progression group was higher than that in control group(MD 180.72,95%CI:116.8~244.65)(P<0.05).Conclusion NGAL is an important biomarker,it can indicate the progression of CKD.
