PFNA与PFLP治疗老年人股骨粗隆间骨折的效果比较

Comparison of PFNA and PFLP in the treatment of intertrochanteric fracture of femur in the elderly

目的比较股骨近端螺旋刀片抗旋髓内钉(PFNA)和股骨近端解剖锁定钢板(PFLP)治疗老年人股骨粗隆间骨折的临床疗效。方法回顾性分析因股骨粗隆间骨折行手术治疗的60例年龄≥65岁的老年患者,按照治疗方法分为PFNA组(n=30)和PFLP组(n=30),所有患者资料均获得完整随访;观察2组患者手术时间、术中出血量、切口长度、住院时间、术后并发症、骨折愈合时间、患髋Harris评分。结果 PFNA组与PFLP组相比,手术时间、术中出血量、切口长度、住院时间前者均优于后者(P<0.05);患者平均随访6个月,PFNA组术后并发症、骨折愈合时间及患髋1、3、6个月的Harris评分均明显高于PFLP组,差异有统计学意义(P<0.05)。结论 PFNA治疗老年人股骨粗隆间骨折的临床效果显著,可减少手术创伤,加快骨折愈合,减少术后并发症,提高髋关节功能,术后恢复更快,近期临床效果优于后者。

Objective To compare the clinical efficacy of Proximal Femoral Nail Antirotation(PFNA) and Proximal Femoral Locking Plates(PFLP) in the treatment of intertrochanteric fractures in the elderly.Methods A retrospective analysis was performed of 60 elderly patients with intertrochanteric fracture of femur who underwent surgical treatment in our hospital. The patients were divided into PFNA group(n=30) and PFLP group(n=30) according to the treatment method,and all patients were followed up completely. The operation time,intraoperative bleeding,length of incision,length of hospital stay,postoperative complications,fracture healing time and Harris score of affected hip of the two groups were observed.Results Comparing with PFLP group,operation time,intraoperative bleeding volume,incision length and hospitalization time of PFNA group were better than those in PFLP group(P<0.05). After an average follow-up of 6 months,the complications,fracture healing time,Harris scores in 1 month,3 months and 6 months of affected hip in PFNA group were significantly higher than those in PFLP group(P<0.05). Conclusion PFNA is effective in treating intertrochanteric fracture of femur in the elderly. It can reduce surgical trauma,accelerate fracture healing,reduce postoperative complications,improve hip function,and recover faster after operation.
