Clinical diagnosis and prognosis analysis of severe patients with novel coronavirus pneumonia

目的探讨新型冠状病毒肺炎(NCP)重症患者的诊治方法。方法对收治的确诊为新型冠状病毒肺炎14例重症患者的临床资料进行回顾性分析。结果患者中位年龄54岁,从发病到病情加重的时间为4~15 d,中位时间为7.5 d。患者主要症状为发热、咳嗽及胸闷气短,部分伴有痰血、肌肉酸痛、腹泻等,胸部CT示肺部病灶较重,且进展迅速。8例行经鼻高流量氧疗,1例行无创呼吸机辅助通气,1例行气管插管有创呼吸机辅助通气治疗,病情好转,无死亡病例。结论患有高血压、冠心病及糖尿病等基础疾病及年龄≥45岁为新型冠状病毒肺炎重症的高危人群,且多在发病后1周病情进展为重症,给予抗病毒、氧疗及营养支持治疗,临床症状均明显改善。

Objective To explore the diagnosis and treatment of novel coronavirus pneumonia(novel coronavirus pneumonia(NCP))in severe patients.Methods The clinical data were retrospectively analyzed in 14 severe patients with novel coronavirus pneumonia.Results The median age of the patients was 54 years old.The time from onset to aggravation was 4~15 days,and the median time was 7.5 days.The main symptoms of thepatients were fever,cough and shortness of breath,and some of the patients were accompanied with bloody sputum,muscle soreness,diarrhea,etc.Chest CT showed that the lung lesions was serious and the disease progressed rapidly.8 cases were treated by nasal high flow oxygen therapy,1 case received the noninvasive ventilator assisted ventilation,1 case received the tracheal intubation with invasive mechanical ventilation,the conditions were improved and there were no death.Conclusion The patients who have basic disease,such as hypertension,coronary heart disease and diabetes,and those age≥45 years old are at high risk of critically ill NCP.Most of the cases will develop into severe disease at 1 week after the onset of the disease.The patients treated with antiviral,oxygen therapy and nutritional support can significantly improved the clinical symptoms.
