摘要
分析不缩短指骨显微术联合术后扶他林乳胶剂对断指病人治疗效果、肿痛恢复程度的影响及对患者手术成活率的影响因素。将我院2018年1月至2019年1月间收治的100例断指患者随机分为观察组及对照组;对照组采用不缩短指骨显微术治疗,观察组在对照组基础上术后联合使用扶他林乳胶剂干预;治疗结束后半年时对患者手治疗疗效进行评估;统计两组再植指成活率、并发症发生情况、不良反应、术后肿痛恢复程度及术后肿痛康复时间,采用单因素及多因素分析影响患者断指成活率的影响。观察组治疗临床疗效均明显优于对照组,且差异存统计学意义(P<0.05);观察组治疗后断指存活率明显高于对照组,且差异存统计学意义(P<0.05);观察组不并发症发生率明显低于对照组,且差异存统计学意义(P<0.05);观察组术后肿胀缓解情况明显优于对照组,且差异存统计学意义(P<0.05),观察组患者术后肿胀恢复时间明显低于对照组,且差异存统计学意义(P<0.05);不同离断程度、缺血时间、受伤类型、静脉桥联、受伤水平、血管形成及保存方法均为影响患者断指成活率的影响因素,且差异存统计学意义(P<0.05);离断程度、缺血时间、静脉桥联、受伤水平为影响患者断指成活的独立性危险因素(P<0.05),受伤类型、血管形成及保存方法为影响患者断指成活的独立性保护因素(P<0.05)。采用不缩短指骨显微术联合术后扶他林乳胶剂对断指病人干预后可显著改善患者治疗效果及肿胀恢复程度,且离断程度、缺血时间、静脉桥联、受伤水平、受伤类型、血管形成及保存方法均为影响患者断指成活的独立性因素。
Analyze the effect of phalanx microsurgery combined with postoperative Futalin emulsion on the treatment effect,recovery degree of swelling and pain in patients with severed fingers,and the affecting factors for the survival rate of finger replantation.One hundred patients with severed fingers admitted in our hospital from January 2018 to January 2019 were randomly divided into an observation group and a control group.The control group was treated with non-shortening phalanx microsurgery,and the observation group was treated with non-shortening phalanx microsurgery combined postoperative Futalin emulsion.Evaluate the treatment effect of the patient's hand therapy half a year after the end of the treatment;count the survival rate of replanted fingers,the occurrence of complications,adverse reactions,the degree of postoperative swelling and pain recovery and the time of postoperative swelling and pain recovery.Univariate and multivariate analysis were used to analyze the affecting factors for the survival rate of finger replantation.The clinical efficacy of the observation group was significantly better than that of the control group,and the difference was statistically significant(P<0.05);the survival rate of severed finger after treatment in the observation group was significantly higher than that of the control group,and the difference was statistically significant(P<0.05);The incidence of uncomplication in the observation group was significantly lower than that in the control group,and the difference was statistically significant(P<0.05);the postoperative swelling relief in the observation group was significantly better than that in the control group,and the difference was statistically significant(P<0.05),The recovery time of postoperative swelling in the observation group was significantly lower than that in the control group,and the difference was statistically significant(P<0.05);different degrees of interruption,ischemic time,injury type,venous bridge,injury level,blood vessel formation and preservation methods All of them are influencing factors for the survival rate of severed finger of the patient,and the difference is statistically significant(P<0.05);the degree of disconnection,ischemic time,venous bridge,and injury level are independent risk factors that affect the survival of severed finger of the patient(P<0.05),the type of injury,blood vessel formation and preservation method are independent protective factors that affect the survival of patients with severed fingers(P<0.05).The use of non-shortening phalanx microscopy combined with postoperative Futalin latex can significantly improve the treatment effect and degree of swelling recovery of patients with severed fingers,and the degree of amputation,ischemic time,venous bridge,injury level,injury type,blood vessel formation and preservation methods are all independent factors that affect the survival of patients with severed fingers.
