TAPP与开放式无张力疝修补术的效果比较分析

Comparative Analysis of the Effects of TAPP and Open Tension-free Hernia Repair

目的对比分析腹股沟疝患者采用腹腔镜疝修补术和无张力疝修补术两种方式进行治疗的临床效果。方法方便选择2018年2月-2019年5月在该院接受手术治疗的46例腹股沟疝患者,通过随机分组法分成对照组(23例)和治疗组(23例)。对照组采用开放式无张力疝修补术进行治疗;治疗组采用腹腔镜疝修补术进行治疗。结果治疗组患者术后不同时间点生活质量和疼痛程度评分的改善幅度大于对照组,差异有统计学意义(P<0.05);手术治疗总有效率达到91.3%,高于对照组的69.6%,差异有统计学意义(χ^2=4.380,P<0.05);术后并发症仅有1例,少于对照组的4例,差异有统计学意义(χ^2=4.100,P<0.05);手术操作时间、术后下床活动时间、住院总时间短于对照组,差异有统计学意义(P<0.05);操作期间出血量低于对照组,差异有统计学意义(P<0.05)。结论腹股沟疝患者采用腹腔镜疝修补术进行治疗,能够缩短操作时间,减少出血量,从而降低术后并发症发生率,迅速减轻疼痛,大幅度改善生活质量,使术后的恢复时间缩短,从根本上使治疗效果和患者满意度得到同步提升。

Objective To compare and analyze the clinical effect of laparoscopic hernioplasty and tension-free hernioplasty for inguinal hernia.Methods 46 patients with inguinal hernia who underwent surgery in the hospital from February 2018 to May 2019 were randomly divided into control group(23 cases)and treatment group(23 cases).The control group was treated with open tension-free hernioplasty,while the treatment group was treated with laparoscopic hernioplasty.Results The improvement of quality of life and pain score in the treatment group at different time points after operation was greater than that in the control group,the difference was statistically significant(P<0.05);the total effective rate of surgical treatment was 91.3%,higher than that of the control group 69.6%(χ^2=4.380,P<0.05);postoperative complications were only 1 case,less than 4 cases in the control group,the difference was statistically significant(χ^2=4.100,P<0.05);operation time,time to get out of bed after operation,total hospital stay were shorter than those in the control group,with statistical significance(P<0.05);blood loss during operation was lower than that in the control group,the difference was statistically significant(P<0.05).Conclusion Laparoscopic hernioplasty can shorten the operation time and reduce the amount of bleeding in patients with inguinal hernia.It can reduce the incidence of complications,relieve pain quickly,improve the quality of life greatly,shorten the recovery time after operation,and fundamentally improve the curative effect and patient satisfaction.
