摘要
目的探讨分析T波电交替对急性心肌梗死患者恶性心律失常的预测价值。方法选择在该院治疗的急性心肌梗死患者79例,选择时间为2017年9月—2019年3月,先通过实验室检查和临床症状对患者进行预测,后在此基础上对患者进行T波电交替检测,对单纯实验室检查联合临床症状与T波点交替的恶性心律失常预测准确率进行比较分析,同时对非恶性心律失常患者和恶性心律失常患者不同导联处的最大值进行比较分析。结果实验室检查和临床症状对患者进行预测,预测准确率为54.17%(13例);在此基础上增加T波电交替检测,预测准确率高达83.33%(20例),相比较单纯的实验室常规检查和临床症状分析,增加T波电交替检查可显著提高预测准确率,差异有统计学意义(χ2=4.593,P<0.05)。非恶性心律失常患者与恶性心律失常患者在V1导联处的最大值,差异有统计学意义(P<0.05);V3、V5导联处的最大值差异无统计学意义(P>0.05)。结论通过对急性心肌梗死患者进行T波电交替检查,可提高恶性心律失常预测的准确性,呈现出较高的预测价值,值得临床推广使用。
Objective To explore the predictive value of T wave electrical alternation for malignant arrhythmias in patients with acute myocardial infarction.Methods A total of 79 patients with acute myocardial infarction treated in our hospital were selected from September 2017 to March 2019.The patients were first predicted through laboratory examinations and clinical symptoms,and T patients were then evaluated on this basis.Alternating wave and electric detection,comparing and analyzing the accuracy of predicting malignant arrhythmias with simple laboratory examination combined with clinical symptoms and T-wave alternation,and comparing the maximum values at different leads of patients with non-malignant arrhythmia and patients with malignant arrhythmia analysis.Results The laboratory examination and clinical symptoms were used to predict the patients.The prediction accuracy rate was 54.17%(13 cases).On the basis of this,T wave alternating detection was added,and the prediction accuracy rate was as high as 83.33%(20 cases).Routine laboratory tests and clinical symptom analysis,and the addition of T-wave alternating examination can significantly improve the prediction accuracy.The difference was statistically significant(χ2=4.593,P<0.05).The difference between the non-malignant arrhythmia patients and the patients with malignant arrhythmia at lead V1 was statistically significant(P<0.05);there was no significant difference between the maximum values at leads V3 and V5(P>0.05).Conclusion Through T wave alternating examination of patients with acute myocardial infarction,the accuracy of prediction of malignant arrhythmias can be improved,and it has a higher prediction value,which is worthy of clinical popularization.
