预防使用泡沫敷料加改良俯卧位通气方式对ICU患者压力性损伤的影响 被引量:7

摘要 目的探讨预防使用泡沫敷料加改良俯卧位与传统俯卧位方式在ICU患者皮肤保护的应用效果。方法选择该院ICU于2017年1月—2019年1月收治需俯卧位治疗的患者30例,采用随机数字表的方式将患者平均分为试验组和对照组,各15例。对照组患者采用传统俯卧位方法,试验组患者采用预防使用泡沫敷料加改良俯卧位方式,比较两组患者的护理效果。结果试验组和对照组研究前PaO2、PaO2/FiO2比较,差异无统计学意义(P>0.05),12 h后两组患者PaO2、PaO2/FiO2均较之前显著改善(P<0.05),试验组上述指标改善效果优于对照组(P<0.05);试验组压力性损伤总发生率为20.00%,对照组的为86.67%,差异有统计学意义(χ2=13.393,P=0.000);对照组胸部的压力性损伤发生率(73.33%)高于试验组(20.00%),差异有统计学意义(P<0.01)。结论预防使用泡沫敷料加改良俯卧位方式在减少皮肤压力性损伤发生上相对于传统俯卧位方式有明显的优势,适于临床俯卧位患者临床应用。 Objective To explore the effect of the application of foam dressing plus improved prone position and traditional prone position on skin protection in patients with ICU.Methods 30 patients who were treated with prone position in the ICU in the hospital from January 2017 to January 2019 were selected,and the patients were evenly divided into a test group and a control group using a random number table,with 15 patients in each group.The patients in the control group used the traditional prone position method,and the patients in the test group adopted the preventive use of foam dressings plus a modified prone position to compare the nursing effects of the two groups of patients.Results The comparison of PaO2,PaO2/FiO2 before and after the study the difference in the experimental group and the control group was not statistically significant(P>0.05).After 12 h,PaO2,PaO2/FiO2 in the two groups were significantly improved(P<0.05);the total incidence of stress injury in the experimental group was 20.00%;the control group was 86.67%.The difference between this index in the two groups was statistically significant(χ2=13.393,P=0.000);The incidence of stress injury(73.33%)was higher than that of the experimental group(20.00%),and the difference was statistically significant(P<0.01).Conclusion The use of foam dressing plus modified prone position has obvious advantages over traditional prone position in reducing the occurrence of skin pressure injuries,and it is suitable for clinical application in patients with prone position.
